SOLAS74 (01) III/14.1 : Rescue Boats shall be stowed in a state of continuous readiness for launching in not more than 5 min. PSCO observed the crew unable to launch rescue boat withing the alotted 5 minutes. The davit arm was unable to hoist the rescue boat.

Oil fuel pipes, which, if damaged, would allow oil to escape from a storage, settling or daily service tank having a capacity of 500L and above shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space. Quick closing valve failed to close inside the emergency generator space.

VESSEL EXPERIENCED A LOSS OF PROPULSION DUE TO A LUBE OIL SENSOR FAILURE ON MAIN ENGINE.
