Towing vessel must carry at least one appropriate-size immersion suit for each person onboard; seams of one immersion suit were deteriorating, repair or replace. Limited to five total POB until rectified. 46 CFR 141.350

Each towing vessel must have a fire-detection system installed to detect engine room fires, ensuring that the design of the system and its installation on the towing vessel is certified and inspected by a registered professional engineer with experience in fire-detection system design, NICET level IV tech, or authorized class society; required certification was not available. 46 CFR 142.330(a)(7)

Each towing vessel must be equipped with navigation lights, shapes, and sound signals in accordance with COLREGS; vessel did not have any dayshapes; acquire minimum of 3 ball and 1 diamond dayshape. 46 CFR 140.720

Electrical equipment must be marked with its respective current and voltage ratings; distrib panels not marked with rated load of each circuit. Label circuit directory with rated load, ensure no breaker exceeds rated load. 46 CFR 143.400

The following list of items must be recorded in the TVR, official logbook: (1) Personnel records, in accordance with § 140.400; (2) Safety orientation, in accordance with § 140.410; (3) Record of drills and instruction, in accordance with § 140.420; (4) Examinations and tests, in accordance with § 140.61(5) Operative navigational safety equipment, in accordance with § 140.620; (6) Navigation assessment, in accordance with § 140.635; (7) Navigation safety training, in accordance with § 140.645; (8) Oil residue discharges and disposals, in accordance with § 140.655; (9) Record of inspection of towing gear, in accordance with § 140.820; and (10) Fire-detection and fixed fire-extinguishing, in accordance with § 142.240. Not all items were readily locatable during inspection; correct by next topside. 46 CFR 140.915

Each towing vessel must be fitted with a general alarm; GA was not operational. 46 CFR 143.235(b)

Vessel was unable to complete Annual Inspection, within 3 months of it's anniversary date, due to crew-availability shortfalls at end of the 2024 season. Vessel is laid-up in Duluth, MN for the winter. Vessel may not operate commercially until a Certificate of Inspection (COI) has been satisfactorily completed (anticipate Spring 2025).

Each liferaft must have a painter system providing a connection between the vessel and the liferaft. The painter for the liferaft is not connected to the vessel.

The owner or managing operator of a towing vessel must establish procedures to address fall overboard prevention and recovery of persons in the water, including, but not limited to use of retrieval equipment. The crew is unfamiliar with procedures for recovery of a person in the water and requires additional equipment to accompany the installed davit.

Flexible connections between cylinders and distribution piping of semi-portable or fixed carbon dioxide fire extinguishing systems and discharge hoses in semi-portable carbon dioxide fire extinguishing systems must be replaced or tested at a pressure of 6.9 MPa (1,000 psig). Dry rot was identified on the flex loop of the semi-portable system in the Fidley. Test to the required pressure or replace in kind.

Machinery must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. One of the steering stop assists is non-operational and allows for overextension of the ram.

The vessel, including crew's quarters and the galley, must be kept in a sanitary condition. The vessel does not have a functioning head or accompanying sink. Re-install prior to overnight voyages.

Each escape hatch and emergency exit used as means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high: “EMERGENCY EXIT, KEEP CLEAR”. The escape hatch in the engine room does not have proper markings.

Conduct hydro static test of air receivers in E/R in the presence of a CG Marine Inspector.

Electrical equipment and electrical cables must be suitable protected from wet and corrosive environments. Install junction box for spliced wiring on flying bridge.

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. Remove dead ended cabling in overhead of upper stateroom.

Electrical conducters passing through watertight bulkheads must be installed so that the bulkhead remains water tight. 07 bulkhead penetrations in upper stateroom to be renewed or made water/fire tight.

An exposed hazard such as gears and rotating machinery must be protected by a cover, guard or rail. Repair broken railing on upper engine room space aft by CO2 bottles.

Sufficent lighting suitable for the marine environment must be provided within crew working areas. Renew broken light fixture globe on aft deck.

Install oversized washers of 1/2" steel plate to seal the bolt openings through the main deck where the winches are mounted.

Starboard side forward cleat shall either be removed or welded to the deck.

In the starboard deck locker (old CO2 space) an electrical cable is improperly spliced. The wiring connection shall be made inside of a junction box.

Renew all air receiver pressure relief valves with appropriate valves with a relif setting not to exceed 250 psig.

The opening in the DC electrical panel is not gaurded to prevent inadvertent access. Install a cover or guard over the opening.
