All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Vessel reported unknown issue with port side gear box. Submit tech report determining cause of the issue and a plan for repairs. Notify local OCMI once issue has been determined and plans for repair.

In order to receive a COI the vessel must conform to all conditions and regulatory requirements applicable to it. The vessel did not have a valid Vessel Security Plan, Merchant Mariner Credentials and TWIC’s, Shipboard Marine Pollution Emergency Plan, Safety management Certificate, Certificate of Financial Responsibility, Navigation Publications. Station Bill, Fire and Safety Plan, Ballast Water Management Plan, Waste Management Plan were not valid/missing. The vessel did not have a crew on board and could not conduct drills. As a result of an absent vessel security plan and crew, the vessel did not receive the ISSC. In order to conduct commercial operations under its issued COI, as a US flagged ship, the vessel will be required to show all documents are present/valid and a credentialed crew is available to conduct drills and demonstrate proficiency.
