Oil fuel pipes, which, if damaged, would allow oil to escape from a storage tank, having a capacity of 500L and above situated above a double bottom, shall be fitted with a cock or valve directly on the capable of being closed from a safe position outside the space. PSCO observed remote valve seized in the open position for the tank supplying the emergency generator. SOLAS 74 (2009 cons. ed.) II-2/4.2.2.3.4

Resolution: The remote valve spun freely because of a broken cotter pin connecting it to the reach rod. Vessel personnel replaced cotter pin which re-connected the reach rod with the remote valve.

The emergency generator automatically started when power was secured to the main generators, however, the final emergency power was not transferred within 45 seconds. Chief Engineer affected repairs in presence of attending MI and was able to demonstrate transfer to the emergency power source an additional (3) times during inspection.
