Machinery and associated piping and fittings shall be of design and construction adequate for the service for which they are intended. Vessel reported a small leak in your No. 4 MDG LT cooler inlet pipe. Temporary repairs have been made which is leak free with no deformities, each watch will inspect and log the status and any changes. Permanent repairs or replacement in kind shall be completed for the No. 4 MDG LT cooler inlet pipe to the satisfaction of the RO (ABS).

Function of fixed fire detection & fire alarm system shall be periodically tested by means of heat or smoke for which the detector is designed. The detection system shall initiate audible & visual alarms on the navigation bridge and engineer officer locations. MI's noted 106 faults on the fire detection panel impacting Zone 2 (Frames 20-30; passageways, stairtowers, laundry rooms, galley, engineer's workshop, and pump room throughout multiple decks spanning the entire vertical zone). Approximately 80+ detectors disabled due to system fault and "no answer" with detection panel for alarm signals. Detection system shall be satisfactorily repaired & tested by authorized service provider prior to movement. Vessel is currently pier-side at Army North Dock in Yokohama, Japan. Resolution: Renewed o-ring on cylinder oil ine and brake line temporary repaired with rector tape. New brake line to be installed at upcoming maintenance period. Tested at working pressure with no leaks.

Fire line / pipe for the AFFF System running in way of the overhead in the Crew Laundry ( 1-18-1 ) developed at leak. Master reported that the fire line was pressured during anchor heaving operations for wash down of anchor chain when the line started to leak. Upon further investigation by the crew and upon removal ceiling panels, it was discovered that the pipe had Two (2) approx. 1 inch long fractures.
Action required: 10 - Deficiency Rectified
