The arrangements for the storage, distribution, and utilization of oil used in pressure lubrication systems shall be such as to ensure the safety of the ship and persons on board. PSCO observed L/O leaking from flow sight glasses on all M/E cylinders. 74SOLA(04)II-2/4.2.3.1
Condition: Improper/Lack of Maintenance
Action required: 40 - Rectify deficiencies prior to next US port after sailing foreign
Resolved 27 June 2022
Resolution: All gaskets of sight glasses were replaced with new ones. There is no leakage and the condition was found in good order.

The company should establish procedures, plans, and instructions, including checklists as appropriate for key shipboard operations concerning the safety of the personnel, ship, and protection of the environment. The various tasks should be defined and assigned to qualified personnel. PSCO observed the crew testing heat detector with a hair dryer. No SMS procedure was provided for testing of detectors.

Resolved 27 June 2022
Resolution: The tester with necessary consumables was supplied and carried out the heat detectors test with satisfactory result. It was confirmed that the ship's SMS "Maintenance plan for Fire-fighting equipment" was revised.
