Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. Vessel had reported experiencing a loss of propulsion to PORT main engine; provide root cause analysis, effect repairs, and conduct satisfactory sea trial. Vessel is prohibited from carriage of passengers. Allowed one time transit to Alameda facility. 46 CFR 119.100

An internal structural examination consists of a complete examination of the vessel's main strength members, including the major internal framing, the hull plating, voids, and ballast, cargo, and fuel oil tanks. A cracked weld was observed below the waterline on hull plating butt joint starboard side void 3 between 6th and 7th inboard longitudinal, just forward of the forward most transverse frame. Conduct NDT to determine if crack extends into hull plating. 46 CFR 115.610(b)

Penetrations in A-Class fire control boundaries for electrical cables, pipes, trunks, ducts, etc. must be constructed to prevent the passage of flame and smoke for one hour. In addition, the penetration must be designed or insulated so that it will withstand the same temperature rise limits as the boundary penetrated... Multiple cable transits on the aft bulkhead of the stbd engine room and lazarette had mixed putty, remove and replace with one putty type. 46 CFR 116.415(a)(2)(ii)

Nonmetallic hose may be used for wet exhaust systems provided it is especially adapted to resist the action of oil, acid, and heat, and has a wall thickness sufficient to prevent collapsing or panting, and is double clamped where practicable. A nonmetallic hose in the port side generator wet exhaust system was not double clamped. 46 CFR 119.430(e)

An internal structural examination consists of a complete examination of the vessel's main strength members, including the major internal framing, the hull plating, voids... Cracked welds observed during ISE to be repaired prior to 2026 credit drydock... 1. Overhead longitudinal in starboard void 3 at the top inboard side of the 2nd transverse swash bulkhead from forward. 2. Port void 2 at a deck stiffener above the first transverse frame.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Vessel reported limited propulsion on STBD engine with no alarms given and lack of prop wash. Provide root cause analysis of propulsion system issue, provide technician report for MI review, and conduct satisfactory sea trials to MI satisfaction. 46CFR119.200(a)

The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located. MI observed port engine room fuel tank valve seized and unable to be operated from the pull outside of the compartment. 46 CFR 119.455(b)(3)

Penetrations in A-Class fire control boundaries for electrical cables, pipes, trunks, ducts, etc. must be constructed to prevent the passage of flame and smoke for one hour. MI observed cable penetrations in port engine room aft bulkhead outboard, starboard engine room aft bulkhead, and under helm console in lower area behind ladder, missing or with inadequate fire-stop putty. 46 CFR 116.415(a)(2)(ii)

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. Vessel operator has stated that the vessel's port main engine is inoperable. Vessel to provide root cause and corrective action to the cognizant OCMI. 46 CFR 119.100

A-Class bulkheads or decks must be composed of steel or equivalent material...they must be so insulated with approved structual insulation. Bulkhead separating lazarette and port engine room missing insulation on engine room side all around access bolts.

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural examination at least once every two years. The vessel has not completed a drydock examination within the last two years. 46 CFR 115.600(d)(1)

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load. During initial operations on 8/19/23, vessel received high temp alarm for port main engine that was found to be due to a closed raw water valve. Operator took the vessel out of service on 8/19 and is working to repair associated damage to port main engine. Diagnose and effect repairs to the satisfaction of the local OCMI prior to carrying passengers.

For semiportable and fixed gas fire extinguishing systems... the owner or managing operator shall provide satisfactory evidence of the required servicing to the marine inspector. No servicing report was available onboard. Provide servicing report to marine inspectors. 46 CFR 115.810(b)(2)

Piping used in a vital system must meet § 56.60 in subchapter F of this chapter: Nonmetallic flexible hose may be used in lube oil, fuel oil and fluid power systems only where flexibility is required and in lengths not exceeding 30 inches. Generator has two fuel hoses connected instead of one flexible hose. Repair fuel hose to be a single length of less than 30 inches. 46 CFR 119.710(c) 46 CFR 56.60-25(b)(4)

A-Class bulkheads...must be so insulated with approved structural insulation, bulkhead panels, or deck covering... Pieces of insulation missing from engine room. Repair fire protection insulation to the satisfaction of the marine inspector. 46 CFR 116.415(a)(2)(i)

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of machinery, fuel, and piping systems, including the following: (a) Operation of the main propulsion machinery both ahead and astern. Salt water leak found on starboard engine. Repair engine to the satisfaction of the marine inspector. 46 CFR 115.804(a)

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the following: (1) Inspection of all accessible parts of the exterior and interior of the hull, the watertight bulkheads, and weather decks. Holes found on deck above both steering spaces. Repair holes prior to next credit drydock. 46 CFR 115.802(a)(1)

Exhaust gas must not leak from the piping or any connections. Exhaust leaking from starboard engine. Repair exhaust leaks prior to next credit drydock. Complete prior to credit drydock. 46 CFR 119.430(b)

The OCMI may require an inspection and testing whenever a repair or alteration is undertaken. Temporary repairs were found on starboard generator exhaust. Make permanent repairs to exhaust system, prior to next credit drydock. 46 CFR 115.700(d)

All electrical equipment and circuits must be clearly marked and identified. Multiple breakers were not labeled. Operator labeled breakers. 46 CFR 120.220(d)

Each vessel must be equipped with at least two operable portable battery lights. One of these lights must be located at the operating station and the other at the access to the propulsion machinery space. No portable lights at operating station or machinery space. Operators procured new portable lights. 46 CFR 120.430

For semiportable and fixed gas fire extinguishing systems...If any of the equipment or records have not been properly maintained, a qualified servicing facility may be required to perform the required inspections, maintenance procedures, and hydrostatic pressure tests. Fire servicing report requires vessel to replace multiple components. Replace all components listed as defective in fire servicing report including Port FM-200 cylinder, cylinder bracket, N2 operated time delay, siren N2 cylinder, both brackets for N2 cylinders additional brackets on main deck, and Starboard FM-200 cylinder. After replacements have been completed, obtain new fire servicing report for replaced components to the satisfaction of the attending marine inspector. 46 CFR 115.810(b)(2) 21SEP23: Changed from 60 to 701 in order to allow vessel to complete engine repairs.

A fire control plan must show escape routes, area of refuge, embarkation stations, compartment titles, and hazard clasification of accomodation and service spaces. Vessel must provide an adequate fire control plan with all appropriate criteria that is approved by the local OCMI.

Approved DVTP's must be completed to the satisfaction of the attending Marine Inspector prior to carrying passengers.

Plans must be submitted and approved for the modifcation to the installed fire extinguishing system that was modified for the engine upgrade.

When using DSC techniques, coast stations and ship stations must use maritime mobile service identities (MMSI) assigned by the Commission or its designees. Vessel has not assigned MMSI to applicable bridge VHF radios. 47 CFR 80.103(b)

Where subject to moisture, a storage cylinder must be installed to provide a space of at least 51 millimeters (2 inches) between the deck and the bottom of the storage cylinder. FM200 cylinder was found sitting directly on the deck. Deficiency rectified on the spot.

At each subsequent inspection, the managing operator shall have the vessel ready for inspection of its fire protection equipment, including testing each firehose to a test pressure equivalent to its maximum service pressure. Vessel's fwd fire hose on the main deck discovered with a pinhole leak. Deficiency rectified on the spot.

Each vessel must have adequate emergency lighting fitted along the line of escape to the main deck from all passenger and crew accommodation spaces located below the main deck. Emergency lights were discovered burned out at the port and stbd fwd main deck EGRESS points. Deficiency corrected on spot.

All escape hatches and other emergency exites must be marked on both sides in clearly legible letters at least 2 inches high "EMERGENCY EXIT, KEEP CLEAR". Port and Stbd escape hatches for the void spaces forward of the machinery space are not labeled on the inside.

AS appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts of large enough scale to make safe navigation possible. The vessel's chart was outdated.

A vessel must carry a first-aid kit approved under approval series 160.041. The vessel's first aid kit was missing aspirin.

The constuction and arrangment of the vessel must allow the safe operation of the vessel in accordance with the terms of the certificate of inspection. Multiple cracks witnessed along forward rails at the bow, and two cracks witnessed at the stbd rail along the aft (stbd quarter) of the vessel. Operator to make appropriate repairs to the satisfaction of the marine inspector.

USCG approved temporary weld repairs have been completed in the stbd machinery space fwd bulkhead adjacent to the stbd fuel tank. Temporary repairs have been proven satisfactory by visual inspection and air lance test. Vessel is authorized to operate with temporary repair until the next dry dock in July 2019. The crew must make a log and visual inspection of the repair twice daily while operating to ensure the continued integrity of the weld.

Each vessel must be provided with bilge pumps in accordance with §56.50-55 in subchapter F of this chapter. Bilge pumps in forward stbd void and stbd lazarette compartments were not properly operating. Pump in forward void could not dewater due to clogged check valve in the same space. Pump in lazarette had a broken seal for the top cover of its housing.

Missing proper structural fire protection where urea pipes pass through engine rooms towards centerline of the vessel.

Design verification and Periodic Safety Test Procedures not approved by MSC.
