No cargo tank hatches, ullage holes, or Butterworth plates shall be opened or shall remain open without flame screens. Vessel's permanent temperature indicating device installation has been altered on every cargo tank. Currently, the vessel has thermocouple sensor cables passing through a hole inside a pipe within the hazardous zone, which by-passes the original connection point, thus allowing cargo vapor to escape through unintended openings and compromising the safety and accuracy of the system while being checked by the tankerman. Vessel shall return to original installation condition of temperature indicating system or prove current installation meets the required level of safety within the hazardous zone. Until such time, vessel is restricted from carrying cargoes with a flash-point below 140° Fahrenheit. 46 CFR 35.30-10 & 35.30-30
