Resolution: Attended vessel and completed IGS testing after receiving a Class survey attesting to the proper installation of the O2 sensor.
1820 - Means shall be provided to control leaks of flammable liquids. PSCO observed on the lower bearing region of both the #1 and #3 cargo pumps some cargo residue that had leaked during a previous discharge operation. Prior to operation of #1 and #3 cargo pumps to the satisfaction of Recognized Organization. SOLAS 74 a (14) II-1/4.1.1. Prior to operation of #1 and #3 cargo pumps.

Resolution: 26APR2016: Received class report from DNV-GL stating that the deficiency had been rectified. The class report stated that the mechanical seals were tightened on #1 & #3 cargo pumps, a function test was carried out and there were no signs of leakage.

PSCO observed that the rubber coupling on the outtake side of the #2 IG blower was cracked around the entire circumference of the coupling. SOLAS 74 a (14) II-1/4.5.5.3.2, FSS Code 15.2.3.1.2.1. 16a.

1099 - The tank overfill alarm required by paragraph (a) of this section must be identified with the legend "TANK OVERFILL ALARM" in black letters at least 2 inches on a white background. PSCO observed that the vessel's overfill alarm tower that was located forward and starboard of the manifold had a weathered legend that was not legible from the aft end of the tower. 46 CFR 39.2007(d). 16c.
