Each item of lifesaving equipment required by this part to be carried on board the OSV must be approved. Vessel placed a Fast Rescue Craft on board that is not approved and is going to be used as a work boat. Vessel is not allowed to utilize the FRC in the capacity as a Rescue Boat IAW 133.135 until is is approved by MSC/[name withheld] 133.40.

No repairs or alterations shall be made to the hull, machinery or equipment that affects the safety of the vessel may be made to the vessel without prior notification to the CG before work starts. Vessel indicated they installed a new davit without prior notification to the CG and without approved plans. Vessel owner/operator must submit and receive approved plans from MSC before continuing the installation.

Three watertight door on the vessel was found not watertight requiring adjustment. Hinge pins in the rudder compartment doors need to be replaced. Vessel shall maintain watertight integrity.

Resolution: Pins changed on the Watertight door from the steering compartment and dogs adjusted. Door confirmed closing properly. Grub screws freed up and dogs adjusted on Emergency exit and engine room entrance doors. Witnessed by ABS to the satisfaction of the MI.

Both Port and Starboard hight jacket-water temperature sensors were replaced during automation testing, the wells were of different type and were not installed. Vessel shall maintain vessel generator's sensors IAW manufacturers procedures.

Resolution: 3. Connections were made on to the CAT sensor for Jacket water temperature. The crew reckon the PT-100 sensor left on top of the generator is a spare one. However all the CAT sensors connected. Over-speed shutdowns on all engines were verified working.

Vessel has a rescue boat and davit installed to conduct ROV operations in order to meet the requirements of 46 CFR 133.135. However, based on the current manning requirements for the vessel, it can not be successfully launched and recovered per 46 CFR 133.160. Therefore, the vessel shall manned with a crew of 05 (five) to include; 1-Master, 1-Mate, and 3-Deckhands while ingaged in ROV operations.
