An alarm system shall be provided indicating any fault requiring attention and shall: Activate an audible and visual alarm on the navigation bridge for any situation which requires action by or attention of the officer on watch. PSCOs discovered the alarm system on the bridge for the engineers alarm inoperable. All other engineer alarm repeaters throughout the vessel worked. 74SOLAS(97 CONS) II-1/51.1.3 16ac

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. In meeting these requirements, the company should ensure that: any non-conformity is reported, with its possible cause, if known. PSCO's observed a hole due to excessive corrosion on the port side bulkhead of cargo hold number 2 on frame #45, above the waterline. It was determined that the master was aware of the damage but failed to report the non-conformity in accordance with the ship's Safety Management System. An ISM audit of the vessel SMS is required before departure. 74 SOLAS (2020) Cons. IX/3.1 ISM Code Part A. 10.1

In the circumstances given in paragraph (b) or where a certificate has expired or ceased to be valid, the officer carrying out the control shall take steps to ensure that the ship shall not sail until it can proceed to sea or leave the port for the purpose of proceeding to the appropriate repair yard without danger to the ship or persons on board. PSCO's observed a hole due to excessive corrosion on the port side bulkhead of cargo hold number 2 on frame #45, above the waterline. Because the condition of the cargo hold is not maintained to the provisions of the present regulations as provided in the SOLAS Cargo Ship Safety construction certificate, it ceases to be valid. 74 SOLAS 20 I/19(c)

Arrangements shall be made to ensure immediate availability of a supply of water from the fire main at the required pressure either by permanent pressurization or by suitability placed remote arrangements for the fire pumps. PSCO's observed that the emergency fire pump was not capable to start remotely, only local. Vessel is required to meet the requirements of Reg. 19.3 per the requirements of exisiting ships, II-2/2.4. 74SOLAS (2020 Cons) Reg. II-2/19.3.1.1 16ac

Detectors and manually operated call points shall be grouped into sections. The activation of any detector or manually operated call point shall initiate a visual and audible fire signal at the control panel and indicating units. Smoke detectors 1-3 and 1-6 located on deck 1 of engine room did not initiate a visual or audible fire signal at the control panel and indicating units. 74SOLAS (97 Consolidated) II-2/13.1.4

Every lifebuoy shall... If it is intended to operate the quick-release arrangement provided for the self-activated smoke signals and self-igniting lights, have a mass sufficient to operate the quick-release arrangement or 4 kg, whichever is greater. Lifebuoys located on both bridge wings were not of the correct weight. PSCO observed the lifebuoys of only 2kg. 74SOLAS (97 Cons.) III/31.7

The means for securing and monitoring weather tightness shall be to the satisfaction of the Administration. The arrangements shall ensure that the tightness can be maintained in any sea conditions. PSCO observed a port side forward weather deck emergency escape hatch was unable to be properly secured to ensure weather tightness. ICLL 66/68 Annex I/ Reg. 16.4

0710 Doors required to be self-closing shall not be fitted with hold-back hooks. According to fire plan, the A-0 door to the galley is required to be self-closing with a magnetic door holder- this door is fitted with a hold-back and does not have the magnetic holding device required by the ship's fire plan. 74 SOLAS (97) II-2/47.2. 50b

1275 Where air pipes to ballast and other tanks extend above the freeboard or superstructure deck, the exposed parts of pipes shall be of substantial construction. Vent pipe from ballast tank (port side, midship) has significant detioration on its top section and the metal is wasted through in a few spots. ICLL '66 I/20.1

MARPOL ANNEX I REG. 15.A.2: Any discharge into the sea of oil or oily mixtures shall be prohibited. Vessel is discharging an oily sheen around controllable pitch propeller and rudder.

SOLAS 74 ('09 CONS) III/20.2: All life-saving appliances shall be in working order and ready for immediate use. Port side inflatable pontoon on vessel's rescue boat was found flat.

SOLAS 74 ('09 CONS) II-2/9.4.2.1: Doors fitted in boundary bulkheads of machinery spaces of category A shall be reasonably gas tight and self closing. (3) Three A class fire doors have been modified with a hold back bracket in the engine space.
