Notwithstanding the requirement of regulation 1/2, paragraph 7, an Administration may, if circumstances require, subject to the provisions of paragraph 1, allow a seafarer to serve for a period not exceeding three months on board a ship entitled to fly its flag, while holding an appropriate and valid certificate issued and endorsed as required by another Party for use on board that Party's ships but which has not yet been endorsed so as to render it appropriate for service on board ships entitled to fly the flag of the Administration. Documentary proof shall be readily available that application for an endorsement has been submitted to the Administration. STCW (2017) Reg I/10.5

STCW CH. 1 Reg. 1/4.1.2 Control excersized by a duly authorized control officer under article X shall be limited to the following: Verification that the numbers and certificates of the seafarers serving on board are in conformity with the applicable safe manning requirements of the Administration. PSCOs observed the vessel is not meeting the Minimum Safe Manning document. The second engineer is rated as a III/1 but is required to be rated as a III/3. 17 bc

MARPOL Annex VI/8 The International Air Pollution Prevention Certificate shall be drawn up in a form corresponding to the model given in appendix I to this Annex and shall be in at least English, Spanish or French. If an official language of the issuing country is also used, this shall prevail in case of a dispute or discrepancy. PSCOs observed IOPP Certificate did not have a supplement. 17 bc

CCSS Code 4.14 Every ship to which this chapter applies shall be provided, with not less than six rocket parachute flares, six hand held red flare signals and two buoyant orange smoke signals. As of July 2024, all of the hand held red flare signals and all rocket parachute flares are expired. 17 bc

PSCO observed that the vessels masters flag state endorsements has expired for both his captains certificate of competency and his GMDSS certificate of competency. STCW Regulation I/10.5

EACH OPERATION DESCRIBED IN PARAGRPH 2 OF THIS REGULATION SHALL BE FULLY RECORDED WITHOUT DELAY IN THE OIL RECORD BOOK PART 1 SO THAT ALL ENTRIES IN THE BOOK APPROPRIATE TO THAT OPERATON ARE COMPLETED. PSCO'S OBSERVED WEEKLY SOUNDINGS WERE NOT CONDUCTED ON 02 OCCASIONS. MARPOL(17)ANNEX I/17.4

All Cargoes... approved by the administration... The cargo securing manual shall be drawn up to a standard at least equivalent guidelines developed by the organization. The approved cargo securing manual on board does not reference any cargos or the method to secure them. 74 SOLAS (14 Cons) Chap 6, Reg 5.6

To enable pipes of reception facilities to be connected with the ship's standard pipeline for residues from machinery bilges and from oil residue tanks, both lines shall be fitted with a standard discharge connection. Machinery space shall be properly formed and efficiently enclosed by steel castings of ample strength and shall be fitted with doors complying with the requirements of regulation 12(1). The port side forward deck hatch cover wll not close rendering the hatch to not be watertight. Improper hinge weld impedes proper function. ICLL 66/88 Annex I/Reg.17.1

Electrical installations should be as such that the safety of personnel and ship from electrical hazards will be assured. Port side ship service generator has water leaking into the generator housing in the vicinity of the windings, significantly increasing electrical shock hazards for the crew. CCSSC CH.2/Reg 2.26.1.3

Windlass should be properly designed to meet all foreseeable operational loads and conditions. Brake tensioning assembly is inoperable, anchor held in place with a turnbuckle. CCSSC Ch. 2/Reg 2.9.2.1

03 - 0740 - CCSSC CHAP 3.3.10.1 AN EMERGENCY FIRE PUMP SHALL BE POSITIONED OUTSIDE THE ENGINE ROOM AND PERMANENTLY CONNECTED TO THE FIREMAINS. THE USE OF PORTABLE FIRE PUMPS SHALL BE AVOIDED. SHIP DOES NOT HAVE FIXED EMERGENCY FIRE PUMP.

1730 - Oily-water Separating Equipment: Oil filtering equipment referred to in paragraph 1 of this regulation shall be of a design approved by the Admin and shall be such as will ensure that any oily mixture discharged into the sea after passing through the system has an oil content not exceeding 15ppm.Oil content meter not reading properly due to a faulty solenoid.MARPOL 73/78(2011)AnnexI/Reg14.6
