Vessel is limited to a rivers route w/in 1nm from shore until an approved survival craft is installed.

Company did not provide adequate relief crew members to vessel when operation exceeded more than 12 hours from the dock as stipulated on the COI. From July 10th to July 11th, company did not provide relief for a deckhand who worked 13 hours in a 24 hour period. Company must provide corrective action plan to Coast Guard for approval. 46 CFR 137.120(a) Based on objective evidence submitted, this def was changed to a 30 day def to allow time for MSU LC to confirm compliance with Sub M over an extended period of time.
