PSCO observed connector to the fuel solenoid valve for the auxiliary boiler had an arrangement in which it was loose and held together with a clamp. 74 SOLAS (04) II-1/26.1

MARPOL ANN 1 REG 17.4 Entries in ORB pt 1 for 12JUN2012 & 13JUL2012 contained inaccurate tank retentions from previous bilge water holding tank entry. 0.5 & 0.6 cubic meter differences were found. Chief Engineer said this was miscalculation on his part, PSC inspectors verified OWS was operated on dates & times of entries.

Resolution: PSC inspectors expanded exam by checking historical OCM data to ensure OWS was run during dates in question, thoroughly examined OWS & all piping systems. Chief Engineer admitted to miscalculating tank levels.
