Where penetrations of watertight bulkheads and internal decks are necessary for electrical cables, arrangements are to be made to maintain the watertight integrity. Roxblocks in the bulkheads entering the engine room and fwd machinery space were improperly sealed as they were drilled through to allow cable penetrations.

Resolution: Received ABS report that indicates vessel received replacement rudder. The rudder was fabricated as a spare at the time the vessel was built. It was installed to the satisfaction of the attending ABS surveyor and rudder operation test was conducted sat.

Each vessel must be designed and constructed to minimize fire hazards; insulation in CO2 room and port machinery emergency escape was wasted. Fire door to stairwell leading to bridge was not self closing. 46 CFR 127.220(a)

The owner or operator of an OSV shall make the vessel available for inspection by a marine inspector each time an accident occurs; Vessel representative indicated that the starboard tailshaft split in half while the vessel was operating offshore. Proceed to Bollinger Amelia, Louisiana to come up on drydock and have starboard tailshaft replaced or renewed as original. Enroute from Port Fourchon, LA to Amelia, LA, vessel is to not carry offshore workers or cargo. Vessel is to notify MSU Morgan City upon arrival inport to make arrangements for attending Marine Inspector to verify completion of all required repairs.

Each fall used with a launching appliance must be renewed either when necessary because of deterioration or after the passage of not more than 5 years which ever comes earlier. Vessels fall was last renewed in June 2014 exceeding the 5 year mark. Replace rescue boat falls to the satisfaction of the attending CG MI.

A Minimum of two two-way portable radiotelephone apparatus for each fire party for fire-fighters communications shall be carried on board. Those two-way portable radiotelephone apparatus shall be of an explosion-proof type or intrinsically safe. Ships constructed before 1 July 2014 shall comply with the requirements of this paragraph no later than the fist survey after 1 July 2018. Provide required two-way portable radiotelephones to fire suits.
