The machinery, boilers, and other pressure vessels and associated piping shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard to hot surfaces. The refractory on the incinerator door is completed wasted, which is signs of flame egress to the bottom and left sides, as there are burn marks present in these areas. 74SOLAS(04)II-1/26.1

Resolution: The damaged/wasted refractory material on the incinerator door, as well as the furnace have been removed - new refractory has been applied/installed.
