No electrical equipment shall be installed in any space where flammable mixtures are liable to collect including those on board tankers unless the administration is satisfied that such equipment is of a type which will not ignite the mixture concerned. PSCO identified 19 cracked lenses in cargo pump room and deck locker.

All Arrangements used for pilot transfer shall efficiently fulfill their purpose of enabiling pilots to embark and disembark safely. The appliances Shall be kept clean, properly maintained, and stowed. Vessels 15 meter pilot ladder was found with damage to the right hand line, one step above the 14 meter step. Also, on the same line on the lowest wooden step.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. Due to heavy weather, the vessel was pushed aground in Lower Mississippi river. Certification issuing authority to attest to the proper operation of steering gear, propulsion machinery and associated equipment, and integrity of the hull in affected area and hull penetration on port side. 74 SOLAS (20) I/II (a) 60 (a)(c)
