Objective evidence indicates there is an SMS failure abord the vessel. BOM manual 4.4.1 requires the master and C/E to report any damage or deficency to company as soon as they are noted. The boiler water leak was not reported to company, exhaust leaks have not been addressed by crew, vessel personnel are not following procedutes for maintence and repair on electrical systems by inaccurately tagging out inoperable AC units in galley and E/R work shop, and inadequate servicing of EEBD'S. 74SOLAS(04)IX/5 ISM 10.1 & 10.2

Electrical insulations shall ensure safety of crew and ship from electrical hazards. SMS Manual 10.5.2 requires warning signs and work permits. When working on electrical equipment PSCO noted electrical equipment AC unit for galley and AC unit for E/R workshop were in the off position and had red electrical tape across them. C/E and electrician stated the units are inoperable and the electrical tape is to ensure that no one turns on equipment. Crew stated there are no lock-out procedures creating a hazard for the crew.

Firefighting systems and appliances shall be kept in good working order. The emergency fire pump was spraying water from the mechanical seal and multiple other leaks were noticed. Standing water and rust were present indicating a long standing deficiency.
