A continuous-reading oxygen content meter should be fitted to the inert gas supply from the equipment and should be fitted with the alarm set at a maximum of 5% oxygen content by volume. PSCO observed oxygen content meter alarm at 7.5% oxygen content. Deficiency rectified.

06 EMPTY CABLE GLANDS FIND ON COMPRESSOR UNITS 1,2,3 WERE FOUND TO BE MISSING THE APPROPRIATE GLAND PACKING.
CARGO TANKS 2, 3, AND 4 HAD DECK LIGHTING MISSING PLUGS RATED FOR THE CERTIFIED SAFE LIGHTING FIXTURE. SOLAS 74 (14) VII/13.1

THE SYSTEM SHOULD BE CAPABLE OF COVERING ALL AREAS MENTIONED IN 11.3.1 WITH A UNIFORMLY DISTRUBTED WATER SPRAY OF AT LEAST 10 I/M3 PER MINUTE FOR HORIZONTIAL PROJECTED SURFACES AND 4 M2 PER MINTUE FOR VERTICAL SURFACES.
CARGO TANK #1 WATER SPRAY SYSTEM WAS BLOCKED. NO WATER COVERAGE WAS PROVIDED, AFTER SOME TIME THE CREW CLEARED THE BLOCKAGE. CORRECTED ON THE SPOT. SOLAS 74 (14) VII/13.1
