The company should establish procedures to ensure the ship is maintained in conformity with the provisions of the rules, regulations, and any additional requirements established by the company. (10.1) The company should identify equipment and technical systems. The sudden failure which may result in hazardous situations. The SMS should provide specific measures aimed at promoting the reliability of such systems. These measures should include regular testing of standby arrangements that are not in continuous use. (10.3)
Shipboard management manual page 796 of 851 (column #44) requires to check alarm actions for fix gas detection every quarter. It took crew approximately 1.5 hours to demonstrate an alarm functioning test of fix gas detection sample point from cargo pump room. Crew provided calibration records on SMS form number FLT-D20CG11 (3/4) but it does not include records of testing alarms. (10.1)
Ships SMS does not outline how crew should test foam monitors, pressure was not sufficient from aft foam monitor by port side superstructure. Crew stated only way to test would be with foam being discharged. (10.3) Later in exam, tested port and stbd monitors with satisfactory pressure without foam discharging. Earlier problem may have been from different pipe diameters.
