A vessel on a limited coastwise route must carry six hand red flare distress signals and six hand orange smoke signals. A red hand flare distress signal may be substituted for any of the hand orange smoke distress signals. All flares on vessel are expired. Provide 12 new flares. 46 CFR 180.68

Remote fuel shutoff stations must be marked in clearly legible letters at least 1 inch high indicating purpose of the valve and direction of operation. Vessels remote fuel shutoff station not labeled. Provide markings for port and starboard engine/generator fuel shutoffs, and ventilation closures. 46 CFR 185.608

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. Vessel is still completing substantial repairs to hull structure, propulsion, and auxiliary machinery systems following grounding incident started on 31 January 2025. Submit documentation (weight-moment calculations and if required deadweight survey procedure and supporting stability calculations) and obtain approval from the Marine Safety Center for modifications and repairs made to the vessel. 46 CFR 176.700(a)

Whenever damage or deterioration to hull plating or structural members that may affect the seaworthiness of a vessel is discovered or suspected, the cognizant OCMI may conduct an internal structural examination in any affected space including fuel tanks, and may require the vessel to be drydocked or taken out of service to assess the extent of the damage, and to effect permanent repairs. The OCMI may also decrease the drydock examination intervals to monitor the vessel's structural condition. Vessel reported to have ran aground during storm. Vessel requires inspection to determine extent of damage. 46 CFR 176.600(d)

A visual and audible alarm must be provided at the operating station to indicate a high water level in normally unmanned spaces. Starboard "fore-peak" electrical bilge switch inoperable, requires replacement and Starboard "lazarette" compartment audible alarm for high water bilge alarm not operational. 46 CFR 182.530(a) 46 CFR 182.100

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: Date of the drill and training; and scenario and training topics. Vessel unable to provide drill/training logs. 46 CFR 185.520(f)

The owner shall submit the following plans,that are applicable to the vessel as determined by the OCMI: On sailing vessels: (i) Masts, including integration into the ship's structure; and (ii) Rigging plan showing sail areas and centers of effort as well as the arrangement, dimensions, and connections of the standing rigging. Provide complete updated 2024 Rigging PMP for vessel. 46 CFR 177.202(b)(12)

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets. Marking not accurate for amount of life jackets stowed. Provide proper marking on exterior of space. 46 CFR 185.604(f)

Each ring life buoy must be marked in clearly legible block capital letters with the vessels name. Vessel name not legible. Provide vessel name on ring buoy. 46 CFR 185.604(b)

A vessel must carry one floating waterlight attached to the ring via corrosion resistant clip with line 3ft to 6ft in length. Line was not 3 through 6 feet and light was not attached using corrosion resistant clip. Provide 3-6 ft line and corrosion resistant clip. 46 CFR 180.71(d)

A visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces. Port side shower high level visual indicator not operational. Provide operational high water indicator light. Rounds shall be made in shower space every 15 minutes until deficiency rectified. 46 CFR 182.530(a)

A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. Bilge pump indicator at helm not operational for starboard forepeak. Provide visual indicator at helm for starboard forepeak bilge pump. 46 CFR 182.530(c)

A pre-engineered fixed gas fire extinguishing system must automatically shut down all power ventilation systems and all engines that draw intake air from within the protected space. Port side generator did not shut down when pre-engineered fixed fire was activated. Provide shutdown for port generator. While deficiency remains outstanding use of port side generator is not authorized. 46 CFR 181.420(a)(3)

Starboard engine leaking diesel fuel from injectors. Provide leak free engine and ensure bilge is cleaned prior to getting underway. 46 CFR 182.100

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Annual inspection completed on 5 Oct 2022 with deficiency issued to provide full rigging inventory of all old and new rigging components, inspections, and rigging details. Ensure to log all inspections in accordance with vessel PMP. Keep copy of PMP on board vessel at all times and submit revised version of PMP to [name withheld] The deficiency from 5 Oct 2022 has not been cleared. Updated and accurate PMP has not been provided. The vessel shall not fly sails after 31 March 2023 with passengers onboard until a third party rig inspection is completed IAW NVIC 02-16. 46 CFR 176.840

Provisions must be made for closing all supply and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. Vent closures do not close. Provide operational closure devices. 46 CFR 182.465

Each vessel that operates beyond three miles from the coastline must have on board a FCC Type Accepted Category 1 EPIRB installed to automatically float free and activate. EPIRB did not pass testing and may not properly activate. Provide EPIRB that can pass test and activate properly. Vessel restricted to three miles from the coastline. 46 CFR 180.64

Each item of survival craft equipment must be of good quality and efficient for the purpose it is intended to serve. 02 Life Floats have tear and foam damage. Make repairs both Life Floats. 46 CFR 180.175

An EAP training program must be conducted for the employer's crewmembers and supervisory personnel. Supervisory personnel must receive at least 60 minutes of training. Personnel in supervisory position has not received supervisory training. Provide proof of EAP training for all supervisory personnel. 46 CFR 16.401

The name of the vessel must be marked in clearly legible letters and numbers on each side of the life float. The number of persons capacity must be marked on each life float. Name of vessel not on both sides of float and number not on float. Provide name and number on life floats. 46 CFR 185.604

All vessels must have navigation lights that are in compliance with the applicable sections of the navigation rules. Port and starboard nav light lens showing sun damage an restrict 2 mile viewing of nav lights. Provide new lens for nav lights. 46 CFR 183.420

All equipment must be maintained in such a manner as to afford adequate protection from machinery failure. Oil leak on port gen at water pump plate. Make generator stop leaking oil. 46 CFR 182.100

Sail Rigging Preventative Maintenance Plan (PMP)must be onboard vessel at all times. PMP not currently on vessel and not currently updated. Provide full rigging inventory of all old and new rigging components, inspections, and rigging details. Ensure all to log all inspections in accordance with vessel PMP. Keep copy of PMP on board vessel at all times and submitted revised version of PMP to [name withheld]. 46 CFR 176.802

The design, materials, and construction of masts, posts, yards, booms, bowsprits, and standing rigging on a sailing vessel must be suitable for the intended service. Vessel's heim joint on head stay failed. Suitable replacement required. 46 CFR 177.330 Mast removed from service. Vessel authorized to operate at power cat until permanent repairs to the head stay are made. Code 17 for head stay remains with vessel only in sail condition until repairs are made, mast installed, and sea trials conducted.

All machinery must be maintained in such a manner to afford adequate protection from causing fire. Fuel line at injector showing small leak. Clean fuel and tighten injectors. 46 CFR 181.100

FCC Safety Radio Certificate and associated radio inspection must be maintained at all times. Missing FCC Safety Radio Certificate. Provide valid FCC Safety Radio Certificate. 47 CFR 80.13

01 Life Float light not functioning properly. Strobe not flashing as frequently as required. Make light operate as designed, 50-70 flashes per minute IAW UL 1196.

Starboard rudder shaft has slow leak/drip. Clean area and provide photo. Conduct hourly round in lazerette space to ensure leak has not increased. Make repairs to seal/packing at next drydock. Complete at next drydock.

Stbd bilge system did not alarm at operating station. Prove correct operation of alarms in accordance with 46 CFR 176.804. Crew must conduct checks of stbd hull bilges every 30min and log while passengers are onboard.

Port and stbd emergency exits were not labeled on deck. Mark in accordance with 46 CFR 185.606.
