Fire doors in main vertical zone bulkheads, galley boundaries and stairway enclosures other than power operated watertight doors and those which are normally locked shall not have hold back hooks not subject to central control station release. PSCO observed fire door in galley boundary with a hold back hook not subject to central control station release. 74 SOLAS 09 II-2-9.4.1.1.4 10C

Fire extinguishing appliances shall be readily available. PSCO observed access door for portable fire extinguisher and fire hose obstructed by casino equipment. 74SOLAS(2009) II-2/10.1.2

The construction of the doors and door frames in 'A' class dividsions, with the means of securing them when closed, shall provide resistence to fire as well as to the passage of smoke and flame equivalent to that of the bulkheads in which the doors are situated. PSCO discovered escape route door in the casino (6.5.2.a) leading to the embarkation area did not fully close.

All life-saving appliances and arrangements shall comply with the applicable requirements of the Code. All life-saving appliances shall be fitted with retro-reflective material where it will assist in detection. PSCO discovered that over 50% of the life jackets onboard the tenders had deteriorated/ wasted retro-reflective material.

Every crew member w/assigned emergency duties shall be familiar w/these duties before the voyage begins. PSCO discovered that stairwell guides were unfamiliar w/basic emergency knowledge during abandon ship drills. Additionally PSCO discovered lifeboat crew member were unfamiliar w/their duties and emergency procedures, to include and not limited to emergency fuel shut off valve location, procedures to engage emergency steering, and location of provisions. 74 SOLAS (20) III/19.2.1

PSCO observed several 'A' class fire doors missing striker plates, and multiple others had frames that were not capable to maintain fire integrity. 74 SOLAS (20) II-2/9.4.1.1.2

The following functional requirements shall be met escape routes shall be maintained in a safe condition, clear of obstacles. PSCO observed pallets of garbage obstructing an escape route in the garbage room. Additionally PSCO discovered a steel gangway blocking escape route to tender station in the vicinity of fire screen door 4.3.14. Another escape route leading from the spa staff area into the weight room was blocked by a spin bike. 74 SOLAS (20) II-2/13.1.2

Fire doors in main vertial zone bulkheads, galley boundaries and stairway enclosures, the approximate time of closure for hinged fire doors shall be no more than 40s and no less than 10s from the beginning of their movement. PSCO observed fire screen door 12.5.07 closed faster than 10s. Additionally, fire screen door 14.3.01 did not latch properly. 74 SOLAS (20) II-2/9.4.1.1.5.2

Structural fire protection, including fire-resisting divisions, and protection of openings and penetrations in these divisions, shall be kept in good order so as to ensure their required performance if a fire occurs. PSCO observed trash cans blocking the closing of the roller shutter fire door at the grill/fryer in the Terrace Cafe. 74 SOLAS (20) II-2/14.2.1.1.1

Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. Portable extinguishers which have been discharged shall be immediately re-charged or replaced with an equivalent unit. A sprinkler head above ironing machinery in main laundry Deck 3 port side was found with an empty sprinkler ampule without color or liquid.

Doors shall be self-closing. Fire screen door 5.1.08 did not latch and double leaf door 4.2.25 did not latch due to improper sequencing of sequence bar. Deck 15 service door located port aft near the emergency diesel generator had a broken hinge and did not latch.

Escape routes shall be maintained in a safe condition, clear of obstacles. Deck 12 starboard aft art loft emergency exit contained obstacles that blocked the emergency exit. Hand sanitizer station was found in stairway near section valve 7.

Fire-fighting systems and applicances shall be kept in good working order and readily available for immediate use. Fire stations on deck 14 port and starboard aft were blocked by stacked lounge chairs.

The public address system shall be clearly audible above the ambient noise in all spaces, and shall be provided with an override function controlled from one location on the navigation bridge. PSCO found the casino office on deck 6 did not have a PA system installed in the space. SOLAS 20 III/6.5.2

Muster and embarkation stations shall be adequately illuminated by lighting supplied from the emergecny source of electrical power. The embarkation light between lifeboats 8 and 10 on the port side was not working. SOLAS 20 III/11

Additional aids for escape shall be provided as necessary to ensure accessibility, clear marking, and adequate design for emergency situatons. FSD 5.4.13 was improperly marked as an emergency exit but is not designated as an emergecny exit on the ship evacuation plan. SOLAS 20 II-2/13

Escape routes shall be maintained in a safe condition, clear of obstacles. Thumb locks deadbolts were found on FSD 1.4.11 and FSD 2.2.02. SOLAS 20 II-2/13

As far as practicable, oil fuel lines shall be arranged far apart from hot surfaces, electrical installations or other sources of ignition and shall be screened or otherwise suitably protected to avoid oil spray of oil leakage onto the sources of ignition. PSCO found supply and return fuel lines on emergency generator missing shielding screens. SOLAS 20 II-2/4.2.2.5.3

99101 - Other (safety in general) - Escape doors from public spaces that are normally latched shall be fitted with a means of quick release...Quick release mechanisms shall be designed and installed to the satisfaction of the administration and in particular not be equipped with any locking device, set screw or other arrangement that prevents the release of the latch when pressure is applied to the release deice. Various doors in primary and emergency escape routes throughout spa area on deck 14 are equipped with thumb twist locks that prevent the doors from opening when pressure is applied. - 74 SOLAS (2014) II-2 / 13.3.2.6.2

0750- Fire doors in main vertical zone bulkheads shall be self-closing. At least six fire screen doors did not close when tested due to issues with the sequencing bars or door malfunctioning. -74SOLAS(14)II-2/9.4.1.1.5

0945- Means of escape shall be marked by lighting or photoluminescent strips at all points of the escape route. Lighting was inoeprable in amidships crew stairwell between decks 10-14. -74SOLAS(14)II-2/13.3.2.5

The steering gear compartment shall include grating or other nonslip surface to ensure suitable working condition in the event of hydraulic fluid leakage. There is no grating or non-slip surface in steering gear compartment in way of the emergency steering gear ciontrol. 74 SOLAS 04 II-1 / 29.13.2

0710-Fire doors in main verticle zone bulkheads, galley boundaries and stairway enclosures other than power operated watertight doors... shall be self-closing and be capable of closing with an angle of inclination of up to 3.5 degrees opposing closure. Fire screen door 12.3.06 did not close when tested. 74SOLAS (09 cons) II-2/9.4.1.4.1

0750-The purpose of this regulation is to contain a fire in the space of origin. The following functional requirements shall be met: fire integrity of the divisions shall be maintained at openings and penetrations. B-15 doors in the spa which are meant to be self-closing are wedged open, detracting from the fire resistant integrity of that boundary. 74SOLAS (09 cons) II-2/9.1.3

0960-Furniture in stairway enclosures shall be limited to seating.. and shall not restrict the passenger escape route. The aft passenger staircase on deck 12 had a display table that added to the fire risk in the stairway enclosure and detracts from the passengers ease of escape. 74SOLAS (09 cons) II-2/5.3.3

0700-Ships of all types shall bew subdivided into spaces by thermal and structural divisions having regard to the fire risk of that space. A category 13 space (deck 3 Forward Hotel Storage) is being used as a category 8 space. 74SOLAS (09 cons) II-2/9.2.1

2020-The company should clearly define and document the master's responsibility with regard to: verifying that specified requirements are observed. Fire drill was not conducted in accordance with safety management procedures. 74SOLAS (09 cons) IX/3.1; ISM Code 5.1.4

0710-Fire Prevention-Cabin Doors in "B" class divisions shall be self closing type. Hold backs are not permitted. Found at least 02 passenger cabin "B" doors that were being held open by the installed carpet in space. Solas (09 CONS) II-2 Reg 9.4.1.2.2

0710 Fire Prevention-"A" Class division openings shall be provided with permanently attached means of closing. Jacques restaurant fire doors have door stops installed which prevents doors from closing. This pertains to fire doors separating restaurant from passenger corridor. Solas (09 CONS) II-2 Reg 9.4.1.1.1

0750 Fire prevention- Furniture in Stairway enclosures shall be fixed, limited to six seats on each deck, of restricted fire risk and shall not restrict passenger escape routes. Found several tables and chairs in crew center stairway on decks 13 and 14. Solas (09 CONS) II-2 Reg 5.3.3

0750 Fire Prevention-Where the contents and the use of a space are such as there is doubt as to its classification it shall be treated as a space within the relevant category having the most stringent requirement. Found offices set up in BOSN stores (CAT 13) and the fan room across from cabin 10049 on Deck 10, containing combustible material as well. 74 SOLAS (09 cons) II-2 REG 9.2.2.3.2.2

0750 Fire Prevention-Where the contents and the use of a space are such as there is doubt as to its classification it shall be treated as a space within the relevant category having the most stringent requirement. Found flammable locker in Spa store on deck 14 containing more than daily use flammable liquids.74 SOLAS (09 cons) II-2 REG 9.2.2.3.2.2

Where necessary, non-conductive matting or gratings shall be provided at the front and rear of the switchboard. Non-conductive matting is missing in the stren thruster room and at the fin stabilizer control panels.

The safety Management System shall be maintained in accordance with the provisions of the code. Passenger cabin evacuation during ships drills were found to be inconsistent with ship's procedures. Evacuation cards were found in all doors however doors on Port side aft deck 10 were found shut with dead bolts open preventing fire integrity of a B-15 door.

Waste recepacles shall be constructed of non-combustable materials with no openings in the sides or bottom. Plastic trash cans used for paper adn plastic waste were found in main galley and numerous spaces throughout ship.
