Double glass B-15 doors in the Observation Library, doors in the crew lounge, crew gym, crew galley and hotel store manager office were unable to self close.

Every crew member assigned emergency duties shall be familiar with these duties before the voyage begins. Stairway guides knowledge and abilities were not consistently demonstrated. Fire teams did not follow shipboard procedures during demonstration of fire drill. Ship unable to account for all personnel missing during drills either excused or otherwise.
