0799 - ON SHIPS CARRYING FLAMMABLE PRODUCTS A WATER SPRAY SYSTEM FOR COOLING, FIRE PREVENTION, AND CREW PROTECTION SHOULD BE INSTALLED TO COVER CARGO TANK DOMES, CARGO LIQUID LINES AND VAPOUR MANIFOLDS AND BOUNDARIES OF SUPER STRUCTURES NORMALLY MANNED. THE SYSTEM SHOULD BE CAPABLE OF COVERING ALL AREAS WITH A UNIFORMALY DISTRIBUTED WATER SPRAY NO DECK WATER SPRAY COVERAGE OBSERVED ON TANK TOPS 1, 2, AND 3 OR PORT AND STARBOARD MANIFOLDS ON INITIAL PSC TEST OF DWS. UPON SECOND TESTING OF DWS AFTER FOUR HOURS OF MAINTENANCE, ALL TANK TOPS AND BOTH MANIFOLDS STILL DID NOT HAVE DECK WATER SPRAY COVERAGE 74 SOLAS (14) VII/13.1 REF 93 IGC 11.3

Resolution: Received class report from LR. Engaged with onboard Marshall Island flag state inspectors. PSCOs re-attended the vessel and witnessed satisfactory operation of deck water spray with proper coverage for all required areas.

2550- THE COMPANY SHOULD ESTABLISH PROCEDURES TO ENSURE THE SHIP IS MAINTAINED IN CONFORMITY WITH THE PROVISIONS OF THE RELEVANT RULES AND REGULATIONS AND WITH ANY ADDITIONAL REQUIREMENTS WHICH MAY BE ESTABLISHED BY THE COMPANY. THE MEASURES AIMED AT PROMOTING THE RELIABILITY SHOULD INCLUDE THE REGULAR TESTING OF EQUIPMENT THAT IS NOT IN CONTINUOUS USE. COMPANY SMS (###) REQUIRES MONTHLY CHECK AND FUNCTION TEST OF DECK WATER SPRAY SYSTEM TO INCLUDE “ISOLATION VALVE, ALL NOZZLES CLEAR AND SPRINKLER TESTED SAT”. RECORDS INDICATED THESE TESTS COMPLETED SATISFACTORILY. LAST TEST OF DECK WATER SPRAY WAS DOCUMENTED AS CARRIED OUT ON 15 JULY 2018, AND NOTED TO BE SATISFACTORY. THE CONDITION OF THE DWS IDENTIFIED IN DEFICIENCY ONE BY PSC EXAMINERS IN THE NORMAL COURSE OF COC EXAM PROVIDES OBJECTIVE EVIDENCE, AS WELL AS, DWS BRING IDENTIFIED AS CRITICAL EQUIPMENT IN THE COMPANY SMS (##), THAT THE PROCEDURES ESTABLISHED ARE INEFFECTIVE. AN INTERNAL AUDIT FOCUSED ON CARGO SYSTEMS IS RECOMMENDED. 74 SOLAS (14) IX/3.1 ISM CODE 10.1 & 10.3

DURING PSC EXAM, PSCOS WITNESSED BOTH TEST OF PORTABLE AND FIXED GAS DETECTION EQUIPMENT WITH SPAN GAS. LABEL ON SPAN GAS INDICATED 50% LEL, HOWEVER ALL TESTS OF EQUIPMENT RESULTED IN READOUT OF 35% LEL. CONSISTENT LEL READINGS PROVIDE OBJECTIVE EVIDENCE THAT SPAN GAS BOTTLE IS INCORRECTLY LABELED. 74 SOLAS (14) VII/13.1 93 IGC 13.6.6

Resolution: Attended vessel with fixed gas detection technician. Verified proper span gas onboard. PSCOs witnessed function test of fixed gas detection equipment from remote location - 20 % and 30 % LEL alarms verified.
