Propulsion machinery, including main engines, reduction gears, shafting, bearings, and electrical equipment and systems, must: (1) Be maintained to ensure proper operation.
Vessel experienced a loss of propulsion while assisting the M/V SEABULK CHALLENGE into berth at Pier 10, Galveston, TX. The vessel lost the port MDE, and returned across the ship channel to G&H's Facility for troubleshooting.
Once determination of cause, and repairs are made to the satisfaction of the TPO (ABS), submit TPO report to MSU Texas City within 72 hours of TPO clearance.
Action required: 17 - Rectify deficiencies prior to departure
