Portable watertight containers for distress flares and smoke signals shall be of a bright color, and containers shall be clearly marked in legible contrasting letters at least 12.7 millimeters (0.5 inches) high: “DISTRESS SIGNALS”. Flares not stowed in container. Stow flares as specified above. 46CFR185.614

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets. Life jacket stowage is not properly labeled. Properly label life jacket stowage. 46CFR185.604(f)

Each rescue boat and ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018 or other standard specified by the Commandant. The arrangement of the retroreflective material applied after March 11, 1996, must be as specified by IMO Resolution A.658(16). Ring life buoy retroreflective material is degraded, replace retrorelfective material on the ring life buoy. 46CFR185.604(i)

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level. The port hull high water level visual alarm is non operational. Repair or replace and prove proper operation of the the port hull high water level visual alarm. 46CFR182.530

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. Remove trash and debris from helm console. 46CFR176.830

At each initial and subsequent inspection for certification the owner or managing operator shall be prepared to test and make available for inspection all items in the ship's outfit, such as ground tackle, navigation lights and equipment, markings, and placards, which are required to be carried by the regulations in this subchapter, as necessary to determine that they are fit for the service intended. Vessel has wrong size hatch cover wrench used to open hatches. Make available onboard the vessel, the correct size hatch cover wrench. 46CFR176.816

The name of the vessel must be marked on some clearly visible exterior part of the port and starboard bow and the stern of the vessel. The hailing port of the vessel must be marked on some clearly visible exterior part of the stern of the vessel. Vessel is not marked on the stern and port side of the vessel. Label vessel as stated above. 46CFR67.123

Vessel must comply with structural design requirements of Lloyds Yachts and Small Craft, ABS Plastic Vessels, or ABS High Speed Craft. Hull shell damage exposing laminate fibers in the skin coat, beneath the gel coat was identified in the following areas: Top of transom, above the waterline, adjacent to previous attempted fiberglass repairs, and starboard bow fiberglass rub rail, approx. 3' in length, located under a rubber rub rail, above the waterline, parallel to the deck. Complete all identified fiberglass permanent repairs IAW materials listed in 46CFR177.410 (b) to the satisfaction of the attending USCG Marine Inspector. 46CFR177.300(c)

During examination propellers shall be made available for inspection and in serviceable condition. (1) blade on the port propeller is damaged. Repair or replace port propeller to the specifications of the outboard engine manufacturer. 46CFR176.610(a)

A vessel that is exposed to salt water...must undergo a dry dock examination and an internal structural examination once every two years. Last dry dock exam was 05NOV2019. Dry dock was due no later than 30NOV2021. Prior to carrying passengers, complete a satisfactory dry dock and internal structural exam.

Vessel failed to provide up to date charts and publications. Vessel must provide proof of equipping vessel with up to date charts and publications.

Vessel failed to properly label location and amount of lifejackets stowed. Label stowage locations with amount and seperate child and adult life jackets.

Life jackets were unserviceable, limited to carrying 36 adult passenger and 6 child until full complement of lifejackets are provided.

Vessel required to have 6 distress signals. Missing 3. Provide satisfactory amount of distress signals.

Emergency fuel shut-off on starboard side is inoperable an did not actuate. Must be repaired.

Portable fire extinguishers have not been serviced in over a year; provide proof of servicing and replace unserviceable fire extinguisher.

Vessel does not have up to date charts and pubs. Provide up to date charts and pubs.

Only 35 adult lifejackets are on board. Do not carry more than 35 people until new life jackets arrive. (46CFR180.71)

Replace retro-reflective tape on forward life ring. (46CFr180.70)

Add two more flares, for a total of 6.

Only one flashlight on board; provide additional flashlight.

Provide copy of latest MIS submission/form.

Provide EAP policy, hotline number, Substance Abuse Professional name and contact number; post on vessel.

The most recent MIS report from relevant consortium was not available for inspection, provide copy of current MIS report

The B-1 portable fire extiguisher inspection was dated March 2014, provide proof of current inspection.

Vessel's AII, BI & BII portable extinguishers are expired. Have the extinguishers inspected or replace with new ones, IAW 46 CFR 176.810(a)(1).

Vessel chart for area of operation is expired, provide updated chart for the area, IAW 46 CFR 184.420(a)(1).

Port navigation light is inop, Repair or replace navigation light, IAW 46 CFR 183.420.

Ammonia inhalants are expired. Replace with updated ammaonia inhalants, IAW 46 CFR 184.710 & 160.041(b).

Waterlight battery is expired, replace with new battery, IAW 46 CFR 199.190(f)(4).

Vessels alcohol testing strips are expired. Replace with new ones, IAW 46 CFR 4.06.
