Inspection of the life-saving appliances, including lifeboat equipment, shall be carried out monthly
using the checklist required by regulation 36.1 to ensure that they are complete and in good order. A report of
the inspection shall be entered in the log-book. Instructions for on-board maintenance of life-saving appliances shall be easily understood, illustrated
wherever possible, and, as appropriate, shall include the following for each appliance: A diagram of lubrication points with the recommended lubricants. PSCO observed several grease fittings that were painted over, on port and starboard life boat launching appliances. PSCO witnessed lowering of the stbd life boat and observed seized sheave.

Resolution: Received, reviewed and accepted class report from NKK stating the deficiency was rectified. Sheaves and grease nipples replaced/maintenance conducted. Lifeboats lowered and hoisted properly. Crew training on maintenance procedures conducted.

Based upon deficiency 01, and objective evidence collected, PSCO recommends internal audit of the companies SMS specifically in regards to the weekly and monthly checks for life-saving equipment completed within three months.
