THE CONDITION OF THE SHIP AND ITS EQUIPMENT SHALL BE MAINTAINED TO CONFORM WITH THE PROVISIONS OF THE PRESENT REGULATIONS TO ENSURE THAT THE SHIP IN ALL RESPECTS WILL REMAIN FIT TO PROCEED TO SEA WITHOUT DANGER TO THE SHIP OR PERSONS ON BOARD. DURING THE INBOUND VOYAGE TO BERTH, THE VESSEL EXPERIENCED AN ABNORMAL CONDITION WITH THE LUBRICATION SYSTEM ON THE MAIN ENGINE NUMBER ONE CYLINDER. AS A RESULT, THE EMERGENCY AUTOMATIC SLOW DOWN OF THE MAIN ENGINE ACTIVATED AND THE VESSEL ANCHORED DOWN IN LIEU OF CONTINUING TO BERTH. SOLAS 74 amd CH.1/11(a)

Audible and visual alarms for gas detection equipment should be located on the navigation bridge, in the control position required by 13.1.3, and at the gas detector read out location. PSCO observed that the vessel's fixed gas detection system does not produce an audible or visual alarm at any point that is required. Vessel’s crew provided a flag state dispensation for fixed gas detection system. However, this dispensation only addresses the detection for the ballast tanks. The vessel's fixed gas detection system serves numerous points including the vessel's compressor room, cargo heater room , and accommodation spaces. This item must be rectified prior to conducting cargo operations.
