Mechanical ventilation inlets and outlets should be arranged to ensure sufficient air movement through the space to avoid the accumulation of flammable or toxic vapors and to ensure a safe working environment. Gas Detection equipment should be so designed that it may be readily tested. Suitable equipment and span gas for this purpose should be carried onboard. PSCO's could not verify calibration or witness proper calibration of the fixed gas sampling equipment. Further, proper span gas as outlined in the Omicron manual could not be located onboard. Cite 74 SOALS 14 II-2/4.2.2.5.4 Connections within the fuel supply and spill lines shall be constructed having regard to their ability to prevent pressurized oil fuel leaks while in service and after maintenance. Observed excessive fuel oil leaks on #2 & #4 main engine fuel injectors fuel pump boosters.

Cite 74 SOLAS 14 II-1/45.1.3 All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner. The newly installed #1 generator breaker leaves a 4mm gap, allowing access to the inside of the switchboard.

Every ship of 100 gross tons and above shall carry a garbage management plan. Plan shall provide written procedures for collecting and disposing of garbage. Vessel has expired span gas onboard dating back to October 2018. Disposal of those cylinders is not covered in the garbage management plan. MARPOL V/ 10.2 50a,c

On ships carrying flammable products a water spray system for cooling, fire prevention, and crew protection should be installed to cover cargo tank domes, cargo liquid lines and vapour manifolds and boundaries of superstructures normally manned. The system should be capable of covering all areas with a uniformly distributed water spray. No water spray was observed from two out of five nozzles and three out of five nozzles had inadequate coverage on tank dome #1, four out of five nozzles were not supplying water spray on tank dome #2, two out of five nozzles were not supplying water spray on tank dome #3, eight out of 11 nozzles were not supplying water spray on the starboard manifold and seven out of the 11 nozzles did not supply water spray on portside manifold.

Fire hoses shall, together with any necessary fittings and tool, be kept ready for use. 05 gaskets on 05 fire hoses located in the engine room were observed to be dry-rotted and crumbled to the touch. 74 SOLAS (2014 Ed.) CH. II-2/1-2.3.1.1.

On cargo ships, each lifejacket shall be fitted with a lifejacket light. One lifejacket light stored in the portside storage container had an expired lifejacket light. 74 SOLAS (2017 Ed) CH. III / 32.2.2.

Cargp ships shall carry one or more totally enclosed lifeboats complying with the requirements of section 4.6 of the Code.
