Resolution: Class report attested to swapped filters and tightened seals on the fuel oil pump. Oil cleaned and no more leages. No further action required.

1730- Pollution Prevention- 15 PPM bilge separators should be capable of producing an effluent for discharge to the sea containing not more than 15 PPM of oil content of the feed supplied to it. During test of the OWS, PSCO observed oil content meter reach 15ppm, alarm sounded and the three-way valve activated. Crew unable to test oil filtering equipment after alarm sounded. In meeting these requirements, the company should ensure that inspections are held at appropriate intervals. The ship's maintenance checklists do not include inspections intervals for oil filtering equipment in accordance with manufacturers instructions. PSCO observed manufacturers maintenance instruction which requires annual cleaning and inspection of filtering media. Date of Aug 2017 marked on OWS. No other record of OWS cleaning or inspection since that date. -74SOLAS(04 Cons.) IX/5 ISM Code 10.2

15 - Safety Management Systems (ISM) › N/A - No Subsystem › Reports of non-conf., accidents & hazardous occur.

Objective evidence discovered during an expanded ISM exam revealed the following non-conformities. These discrepancies, included with the other material deficiencies, are evidence the ship/company is not implementing the requirements of the ISM Code through the approved SMS procedures. An external audit is recommended. SOLAS 2014 IX/3.1 ISM Code 1.3 The responsibility and authority of the designated person or persons should include monitoring the safety and pollution-prevention aspects of the operation of each ship and ensuring that adequate resources and shore-based support are applied, as required. The crew reported the excessive engine room leaks to the DPA on September 11. The DPA replied on September 12 that supplies would be delivered at the next port after overhaul of AE1 (est. 6-8 hours). The vessel has since called on four ports over seventeen days and no supplies have been delivered to address the identified hazardous condition in the engine room. Adequate resources and shore-based support are not being applied, as required. SOLAS 2014 IX/3.1 ISM Code 4 The company should ensure that the master is: fully conversant with the company’s safety management system. The SMS requires that the master report safety/environmental/security issues via various emails. The master did not use the email address and instead attempted to address the issue via the electronic NP5 program onboard, which is an internal messaging system usually used for “work orders, standard jobs, parts or items, equipment and so forth.” The company may have not recognized the urgency of the request because an improper medium was used to convey it. SOLAS 2014 IX/3.1 ISM Code 6.1.2 The company should establish procedures, plans and instructions, including checklists as appropriate, for key shipboard operations concerning the safety of the personnel, ship and protection of the environment. The various tasks should be defined and assigned to qualified personnel. The crew member assigned the responsibility of checking lifeboat provisions marked the relevant monthly checklist for three months that all provisions were accounted for. However, during the examination of the lifeboat (certified for 32 persons), only 18 food rations were onboard. The lifeboat was missing 14 food rations, and there were 19 crew members onboard. SOLAS 2014 IX/3.1 ISM Code 7 The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. The current condition of the engine room is indicative a lack of preventative maintenance to ensure a safe working environment. SOLAS 2014 IX/3.1 ISM Code 10.1

09 - Working and Living Conditions › 092 - Working Conditions › Cleanliness of engine room

The purpose of this regulation is to prevent the ignition of combustible materials or flammable liquids. For this purpose, the following functional requirements shall be met: means shall be provided to control the leak of flammable liquids. There is fuel oil running in streams on the main engine, there are active fuel leaks on the running generator and jacket water leaks on the main engine, the bilge is coated in wet oily waste with several oil saturated rags floating within, and the deck plates in the lower deck of the engine room are slippery with oil. PSCOs in Martinique wrote a deficiency for the similar issues on 10 September 2018, and no apparent corrective action has been taken since then. The condition represents a severe fire hazard. 74 SOLAS 2014 II-2/4.1.1

Resolution: The vessel addressed the oil running streams on the main engine and generator. Bilges were emptied of rags and oily waste. DNV-GL was made aware of the deficiencies identified in Martinique.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons onboard. The vessel lost propulsion while operating astern, and the crew was unable to restart the engine. Attending tug boats brought the vessel to the pier.

The normal equipment of every lifeboat shall consist of food rations of each totaling not less than 10,000kj for each person the lifeboat is permitted to accommodate; these rations shall be kept in an airtight packaging and stowed in a watertight container. Fourteen of the thirty-two rations were damaged by water. The vessel has nineteen people onboard and only eighteen of the rations were viable.
