PSCOs observed numerous lights in hazardous locations on deck and in cargo tunnel that are no longer safe for use in a flammable atmosphere due to cracks and loose fittings. Deficiency to be rectified prior to cargo operations. IBC CODE 15.1.5, 17AC

Resolution: The relevant lights on the deck have been replaced by the spares and the lights in the tunnel have been isolated, which will be replaced once the spares are available. Risk assessment is available and discussed with the crew who entering pipe tunnel.

The company and its ship shall comply with the requirements of the international Safety Management Code. During inspection of fixed gas detection system, the [name withheld] was not familiar with testing procedures. No logs for testing fixed gas detection was provided. [name withheld] was unable to provide any procedures regarding testing of the fixed gas detection system. Objective evidence exists that the vessel is not implementing the SMS. The vessel has demonstrated a systematic lack of maintenance for critical equipment. Safety management audit shall be carried out by the RO before the ship is released from detention.

Resolution: Correct 02 analyzer has been installed and approved by classification sociey. Operational test of IGG was witnessed by both class and attending Coast Guard exam team.

The company and its ship shall comply with the requirements of the international safety management code. Deck instruction manual 10.1 stipulates that a record of operation and maintenence of inert gas system must be kept on board. Instructions on IGG local panel stipulate that upon start up, calibration shall be conducted and logged. Logs for calibration could not be provided and calibration procedures were for equipment that had been replaced by a different brand of oxygen analyzer. Chief Engineer was not familiar with testing procedures or requirements. A safety management audit shall be carried out by the RO before the ship is released from detention.

Resolution: ISM Auduit was conducted. Proper policys and procedures have been issued. Operational test of IGG was succesful and all procedures were followed IAW SMS.

The inert gas system shall be designed, constructed, and tested IAW the FSS Code. FSS Code states that an audible and visual alarm shall be provided to indicate oxygen content in excess of 5% by volume. During testing of IGG, PSCOs observed oxygen content in excess of 20% being delivered to IG line with no alarms sounded.

Resolution: Proper class approved O2 analyser was installed by technician. IGG operational test was observed and all test and alarms witnessed satisfactory.
