On ships carrying flammable products a water-spray system for cooling, fire prevention and crew protection should be installed and be capable of covering all areas exposed on the cargo tank domes, cargo liquid and vapor discharge and loading manifolds and the area of their control valves and any other areas where essential valves are situated. PSCOs observed that the starboard water-spray nozzles at the transfer manifold did not spray nor did seven nozzles on the #1 and #2 cargo liquid domes.

The cargo containment system should be capable of withstanding the full vapor pressure of the cargo under conditions of the upper ambient design temperatures irrespective of any system provided for dealing with boil off gas. The #4 liquid dome secondary level gauge housing had an incomplete weld allowing gas to escape from the tank to atmosphere. Isolation valve was operational and was able to secure the leak. Use of the gauge is prohibited from use until fixed to the satisfaction of the RO.
