The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. In meeting these requirements, the company should ensure that: inspections are held at appropriate intervals; any non-conformity is reported, with its possible cause, if known; appropriate corrective action is taken; and records of activities are maintained. Objective evidence discovered during the PSC examination revealed the following non-conformity which contributed to a serious systematic failure and lack of implementation of the SMS policy onboard in the areas of training, familiarization, record keeping, and maintenance. Page 53 of the vessel's training manual has a time requirement and training standard of two minutes. During the abandon ship drill, it was discovered that crew members had not been adequately trained in the use of the immersion suits onboard the vessel. No crew members were able to properly don the onboard immersion suit and required life jacket in two minutes. After further training and instruction, crew demonstrated proper donning of immersion suits and life jackets, but exceeded training manual time by six minutes. Vessel was unable to provide documentation that abandon ship drill included immersion suit and life jacket combination required by SOLAS 74 was conducted.
