In meeting these requirements, the company should ensure that inspections are held at appropriate intervals; any non-conformity is reported, with its possible cause, if known; appropriate corrective actions is take; and records of these activities are maintained. Objective evidence during an expanded ISM exam revealed the vessel failed to follow their Safety Management System regarding reports and analysis of nonconformities (Chapter 9) and planned maintenance on vital machinery systems. Long standing non-conformities throughout the engine room have not been reported as required and machinery maintenance is overdue. Means of control for flammable liquids in the space shall be provided. Observed the remote quick closing shut-off valve to the diesel oil service tank blocked open with two bolts, severely limiting the vessel's ability to control fire growth in the machinery space in an emergency. 74SOLAS(09)II-2/5.1.2; 30abc

Means shall be provided to control leaks of flammable liquids. Observed active fuel oil leaks from the diesel oil settling tank valve and fuel oil line on the main burner assembly for the auxiliary boiler, creating a significant fire hazard. 74SOLAS(09)II-2/4.1.1; 30abc.

The proper securing of anchors and cables by stoppers or other satisfactory arrangements. Observed the port side anchor brake severely wasted. The vessel was using an additional securing mechanism in order to keep the anchor from inadvertently deploying. ILO-147/PWSA 46USC70002; 60ac

The physical condition and proper storage of mooring lines and ropes to ensure that they are free of damage and defects and are not exposed to conditions which could lead to their deterioration and eventual failure. Observed several mooring lines that were severely deteriorated. ILO-147/PWSA 46USC70002; 60ac

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship of persons on board. Observed the oil mist detector for the main engine with an active alarm (code E012) indicating an electronic defect. 74SOLAS(14)I/11a; 17ac

The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. Observed missing motor covers on both lube oil purifiers and fuel oil purifiers presenting danger to persons working around the machinery. 74SOLAS(04)II-1/26.1; 17ac

All electric cables and wiring external to equipment shall be at least of a flame-retardant type and shall be so installed as not to impair their original flame-retarding properties. Observed cables and wires on the boiler main burner assembly being held up with zip ties. Additionally, several wires had melted outer jackets due to falling onto the burner assembly. 74SOLAS(04)II-1/45.5.2; 17ac

The means for securing and maintaining weathertightness shall be to the satisfaction of the Administration. Observed missing securing devices on 6 of 7 cargo hold hatch covers. ICLL 16.4; 17ac

Fixed fire-detection and fire alarm installations shall be suitable for the nature of the space, fire growth potential and potential generation of smoke and gas. Observed a plastic bag tied over the detector in the purifier room due to an abundance of a smoke in the space for unknown reasons. 74SOLAS (09)II-2/7.1.1; 17ac

All air cylinders for breathing apparatus shall be interchangeable. While running Fire drill. PSCO Observed two different configurations of breathing air cylinders for breathing apparatus. 74 SOLAS (04) II-2/10.1/ FSS Code 3.2.1.2.1

0725-Fixed fire extinguisng installation. Fire protection systems and fire fighting sytesm and appliances shallb e maintained ready for use, and shall be properly tested.PSCO discovered that the supply water for the hypermist was empty. 74SOLAS(14)II-2-14.1

0925- Musters and drills. Enclosed space rescue drill should be planned and conducted in a safe manner, accounting for guidance provided by the organization. PSCO noted tha the enclosed space rescue drills didn't include atomspheric testing during or prior to entry. 74SOLAS(14)19.3.6.2.3

0715-Detection. Fire protection systems and firefighting sytems and appliances shall be properly tested and inspected. PSCO witnessed crew testing flame detector with an open flame and could not provide evidence that this was an approved method. 74 SOLAS (14) II-2/14.1

0699-Other (life Saving appliances)Emergency lighting for mustering and abandonoment shall be tested each abondon ship drill. PSCO observed vessel was not conducting emergency lighting test with abandon drill. 74SOLAS (14) III/19.3.3.9

1070-UMS Alarms. The conidtion of the ship and its equipment shall be maintained to conform with the provisions of the present regulations. PSCO observed active system faults on the ships UMS manning alarm pannel. Ship is currently manning engineering spaces 24 hours a day but is authorized to shift to periodically unattended. 74SOLAS(14)I-11

SOLAS 74 (09 CONS.)II-2 Reg. 10.2.1 Ships shall carry at least two fire-fighter's outfit. Vessel has only one serviceable fire-fighter's outfit. 17a,c.

SOLAS 74 (09 CONS.)I Reg. 11(b) After any survey of the ship under regulations 7,8,9 or 10 has been comlpeted, no change shall be made in the structural arrangements, machinery equipment and other items covered by the survey, without the sanction of the administration. Observed pin hole leak in cool water piping system in conjunction with a number of soft patches throughout.
