PSCO observed pipe penetration without fire sealant surrounding it and a separate hole approximately one inch in diameter in the crew staircase near fire station 12 both of which were not in accordance with the approved fire control plan and the Class A boundary requirements.

PSCO observed the fire doors on the Spar deck leading to the deack above on the port side near fire station 12 was being held open and subsequently would not latch completely. The fire door exiting from the Engine Room would not close or latch completely. Per the approved fire control plan, both doors are to be self closing and A Class.

33 CFR 160.216-40AC-Vessel has recently reported that the starboard engine has a crack in the cylinder liner and the vessel is transiting at reduced speed on one engine. Vessel is authorized to transit to Motor City Dock in the Detroit River to offload and then proceed to Canada to effect repairs. After returning to Canadian waters and prior to re-entry into U.S. waters, the vsl shall provide for class attendance to confirm repairs as original to the stbd main engine. If continued transit on one main engine is desired vsl shall coordinate with sector detroit for approval to operate within U.S. waters.

1410 - Propulsion Main Engine - All installations shall be of marine type engines suitable for the intended service, design and constructed in conformance with the requirements of this subchapter.
The Port main engine suffered a crank shaft failure resulting in the vessel operating solely on the Starboard engine. The vessel may operate under reduced propulsion for 60 days in accordance with D9INST 16711.1, as long as the vessel is capable of maintaining safe speed and steerage for prevailing or anticipated conditions. The vessel must also have a fully functional bow thruster, two fully operational generators, and the Master and Chief Engineer must provide Sector Detroit with a written statement attesting to the vessel's capability to maneuver properly as well as the suitability of the reduced propulsion machinery to generate thrust.
