In general, compliance with the standards established by the American Bureau of Shipping, see subpart 70.35 of this sub-chapter, will be considered satisfactory evidence of the structural efficiency of the vessel. Make permanent weld repairs as discussed to the deck and framing on the #1 end in the steering space.

Each pump shall be capable of delivering water simultaneously from two highest outlets at a Pitot tube pressure of approximately 50 psi... MI observed loss of pressure at nozzles due to biofouling in fire main system. Vessel to flush system frequently until foreign growths are purged from system and retest in the presence of an inspector at next quarterly.

Each hand portable fire extinguisher shall be marked with a number and the location where stowed shall be marked with a corresponding number at least 1/2" high. Portable extinguishers 8 and 9 not appropriately marked.

Electrical installations on vessels must ensure: Maintenance of services necessary for safety under normal and emergency conditions. Vessel report 1 of 2 Ship's Service Diesel Generators shutdown unexpectedly. Vessel to determine root case, repair, and conduct operational test to satisfaction of MI. 46 CFR 111.01-1(a)(1)

All doors, except those that are kept normally closed, shall be of a type which are capable of release from the control station and from a position at the door. The release mechanism shall be so designed that the door will automatically close in the event of disruption to the control system; however, approved power operated watertight doors will be considered acceptable for this purpose. Holdback hooks, or other means of permanently holding the door open, not subject to control station release, will not be permitted. When double swing doors are permitted, they shall have a latch arrangement which is automatically engaged by the operation of the door release system. MI observed during fire screen door release test, fire screen doors 5, 15, and 18 failed to close and latch.

No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection. Drawings of alterations shall be approved before work is started, unless deemed unnecessary by the Officer in Charge, Marine Inspection. Submit repair proposal for #2 End bow for damage sustained 02AUG2024 following allision at Vashon Island terminal. Increase periodicity of checks made to #2 steering gear compartment.

Fire screen door FSD03 did not close during tests from the control station on the bridge or local position. FSD03 is in the passenger stairway enclosure.

Significant gaps in lagging on exhaust header for main engines.

Explosion covers on main engines installed incorrectly (some upside down).

Boiler circulation pump wiring was isolated by using wire nuts and electrical tape.

An intervening stop valve has been installed in the piping between the fired pressure vessels (#1 & #2 Hot Water Heating Boilers) and the safety relief valves. Lockout/tag-out boilers until the stop valves can be removed. Complete removal by 04 September 2023.

Hot water heating boilers have valves installed between the fired pressure vessel and the low water limit control. ASME/ANSI CSD-1 states: "If the low-water fuel cutoff is connected to the boiler by pipe or fittings, no shutoff valves of any type shall be placed in such piping. Lockout/tag-out boilers until the stop valves can be removed. Complete removal by 04 September 2023.

Where in this subchapter an item, or method of construction, or testing is required to meet the standards established by the American Bureau of Shipping, the current standards in effect at the time of construction of the vessel, or otherwise as applicable, shall be used. The current standards of other recognized classification societies may also be accepted upon approval by the Commandant. Stern tubes are wasted beyond allowable standards. Provided repair proposal for stern tubes on both number 1 and number 2 ends.

Short lengths of nonmetallic flexible hose are being used as vibration isolators for various sensors including fuel and oil pressure on the propulsion engines. Hose end fittings do not comply with SAE J1475. Likewise hose does not meet SAE J1942. Provide hose assemblies that comply with 46 CFR 56.65-25(b).

Various fire screen doors in vicinity of deck crew galley are provided with hold back hooks that cannot be released remotely. Remove hold backs from fire screen doors in the deck crew accommodation spaces. See 46 CFR 72.05-25(b)(9)(ii).

Emergency generator space not readily accessible from the open deck. Emergency hatch leading to the open deck only operable from inside the space. Make emergency generator space readily accessible form the open deck. See 46 CFR 112.05-5(d)

Submit a qualitative failure analysis to indicate automation assumptions, vessel/equipment, operating conditions, failures considered, cause and effect relationships, method of crew detection of failure and alternatives available to the crew of the ship's service and emergency switching gear. Provide a corrective action for any single point failures (non-concurrent) identified, updated and submit the required plans for approval with in 60 days. Any modifications or equipment replacement needed to effect the corrective actions shall be carried out at the vessel's next dry dock and demonstrated via design verification tests to the satisfaction of the attending Marine Inspector. There were numerous faults on the fire detection and alarm panel located on the bridge. Required to clear faults and test the system (ie smoke detectors) prior to departure. 46 CFR 76.27-10

The station license on board the vessel was expired. Required to provide a valid document prior to departure. 47 CFR 80.403

All passenger ships regardless of size must be provided with: A VHF radio installation capable of transmitting and receiving:(i) DSC on the frequency 156.525 MHz (channel 70), and it must be able to initiate the transmission of distress alerts on channel 70 from the position from which the ship is normally navigated; and Radiotelephony on the frequencies 156.300 MHz (channel 6), 156.650 MHz (channel 13), and 156.800 MHz (channel 16). Required to demonstrate distress alert sent through VHF-DSC capable marine radio on each end with GPS position and MMSI. 47 CFR 80.1065 47 CFR 80.1085 47 CFR 80.1087

The radio installation must be clearly marked with the call sign, the ship station identity and other codes as applicable for the use of the radio installation. Required to provide this information on/ near the radio equipment. 47 CFR 1083 (b) (5)

Vessels of 1,000 gross tons and over shall have permanently exhibited for the guidance of the officer in charge of the vessel the fire control and safety plan. The plans within the pilot houses were not the approved final plan and the dates on the bottom of the pages did not match. Discussed and agreed that plan could be kept in drawer as long as the plan is easily accessible and can be quickly referenced. Required to fix. 46 CFR 78.45-1

Provide an emergency means to communicate. The sound power phone located in the E-Gen space index was damaged and undeniable preventing the ability to call a designated space. Qualify all SSP on the vessel. 46 CFR 113.30-25(c)(3)

Repair the leaking CO2 piping located in the branch line feeding the alarm in the uptake space. 46 CFR 76.15-15

Provide a means to isolate the E-Gen space in the event of a fire. The installed auto-dampers did not actuate close during the CO2 test. 46 CFR 76.15-35

Repair the inoperable engineering alarm light in the e-gen space. 46 CFR 62.25-20(d)(2)

Properly mark the GA in the e-gen space to ensure visibility during all operating conditions.

Repair the removed structural fire protection insulation in the e-gen space. 46 CFR 72.05-10

Qualify the label or the breaker for the steering gear. The amp ratings do not match for P412 located in the EOS. 46 CFR 111.30-15

Prove the proper operation of the fire detection system.

Repair or replace the in accurate MDE1 RPM gauge located in EOS. 46 CFR 62.25-20

An alarm should not prevent annunciation of subsequent alarms because of previous silencing. The AMS requires until the watch-stander clears the alarm it prevents a subsequent alarm from annunciating. 46 CFR 62.25-20(e)(viii)

AMS alarms must be clearly distinguishable. In accurate alarms or displayed for various conditions. A low J/W pressure displayed engine shutdown but the engine remained on-line. 46 CFR 62.25-20(d)(1)

An automatic operation of a safety control must be alarmed. The genset's low L/O shutdown did not indicate an alarm condition. 46 CFR 62.25-15(c)

All alarms must have a manual acknowledgment device. The steering alarms in EOS continually alarm even after the silenced button is depressed. 46 CFR 62.25-20(e)(i)

The reverse power relay for the #1 SSDG failed to actuate IAW approved PSTP's. Each generator arranged for parallel operation must have reverse power or reverse current trips. 46CFR111.12-11(f)

Repair #1 PH embarkation light panel. The lights continually flash. 46 CFR 111.01-1(a)(2)

46 CFR 199.178(a) Containers, brackets, racks, and other similar stowage locations for lifesaving equipment must be marked with symbols in accordance with IMO Resolution A.760(18) indicating the device stowed in that location. Check all lifesaving stowage locations and ensure that the proper placarding is posted where missing.

46 CFR 111.12 / 16c - Each generator prime mover must have an overspeed device. #2 SSDG is tripping off line due to nuisance overspeed alarm although actual operation of generator is within normal operating parameters.

FSD #17 does not fully close when released. Master to designate a crewmember to physically close the door in the event of an emergency. 46CFR72.05-25(b)(9)(i)

Properly install light bulbs in vicinity of #2 end times 3 & #2 end times x1 in the passenger cabin area.

Crew unfamiliar with fire fighting fixed CO2 system when instructed to utalalize the CO2 fixed system, crew attempted to use portable CO2 system.

The emergency power source must supply power for twice the time of the run and have the capacity to supply all loads that are simultaneously connected to it. Emergency generator switch board is inop. Vessel not authorized to carry passengers. 46 CFR 112.05-5

If a lever type control is provided, it must be installed and marked so that its movement clearly indicates both the direction of the rudders movement, and if follow up control is provided, the amount of rudder movement, current arrangement has no markings on opposing helm levers.

Each vessel must have a steering failure alarm that actuates an audible and visible alarm in the pilot house when the actual position of the rudder differs more than 5 degrees from rudder position ordered by the follow up controls. In cruising mode the rudder lever was in the port 10 degrees position and no alarm was activated.

Markings in the pilothouse must not interfere with the helmsman's vision, but must be clearly visible at night, does not have illumination to read lever position.

Nozzles at fire station #9 and #23 were clogged with marine detritus.

Emergency generator room natural ventilation supply did not secure during fixed CO2 actuation.

Fire station #1 nozzle fully clogged and others found to be partially clogged.

Replace valves in steam boiler system with appropriate temperature rated valves. 46 CFR 56.20-1(b)

Natural supply vent damper on EDG must close when CO2 discharged in space. 46 CFR 76.15-35

E/R supply fan #2 breaker doesn’t trip when FR Vent circuit tripped. 46 CFR 111.103-3

Number 1 rescue boat release mechanism is bent causing rat nest of wire in the drum. Must repair launching appliance to design specs and spool out wire to inspect for damage and reset proper lay.

Locks on the racks where AFFF is stored on #1 and #2 end are rusted closed. Make fire fighting equipment readily available.

Fire Screen Door #18 does not close when release is actuated.

Provide proof of valid VGP.

Properly label fire control plan.

Fire manifold gauges on the bridge not operational. Prove proper operation.

WTD 001 LOP alarm light malfunctioning. Prove proper operation.

Replace No. 2 MDE grid cooler.
