Each liferaft or group of liferafts shall be stowed with a float free arrangement complying with requirements of 4.1.6 of the Code so that each floats free. Port side liferaft hydrostatic release unit system no installed correctly, with necessary equipment to install properly not onboard. SOLAS 74 (2014 Cons.) III/Reg. 14.4.2

A minimum of two two-way portable radiotelephone apparatus for each fire party for fire-fighter’s communication shall be carried on board. Those two-way portable radiotelephone apparatus shall be an explosion proof type or intrinsically safe. Vessel does not have dedicated suitable radios for fire-fighting team. SOLAS 74 (2014 Cons.) II-2/10.4

There shall be available at all times, while the ship is at sea, a supply of electrical energy sufficient to operate the radio installations. PSCO observed batteries for portable VHF radios were expired. Delivered to ship when PSC team instructed SOLAS 74 (2014 Cons.) IV/13.1

Other (Safety In General) - Each transfer pipe system including each metallic hose must not leak under static liquid pressure at least 1.5 times the maximum allowable working pressure. The frequency of the test required by this section for vessels must be annually or part of a biannual or mid-period inspection. The vessel could not produce record that the transfer piping system had been hydrostatically tested. Provide prior to bunkering. 33 CFR 155.720

Other (Safety In General) - Unless operating exclusively on voyages between ports or places within a single COTP Zone, the master, owner, operator, agent, or person in charge of a vessel subject to this subpart and this section must submit a ballast water report to the National Ballast Information Clearinghouse (NBIC) by electronic ballast water report format using methods specified at NBIC's Web site. Instructed Master and [name withheld] of requirement. [name withheld] is submitting as required to NBIC. 33 CFR 151.2060

nternational Oil Pollution Prevention Certificate (IOPP) - An International Oil Pollution Prevention Certificate (IOPP) shall be issued in accordance with the provisions of regulation 6 of this Annex to any ship of 400 gross tonnage and above. Such certificate shall be endorsed as appropriate either by the Administration or by any persons or organization duly authorized by it. PSCO observed vessel's IOPP Supplement (Form A) states that the vessel is marked to have separating/ filtering equipment and an oil content meter meeting Resolution A.393(X). PSCO observed in the separator manual that the system type of Resolution MEPC. 60(33) MARPOL 73/78 2011 Cons. Annex I/Reg.6

Other (Safety In General) - The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship, or persons on board. PSCO observed that an emergency light next to the starboard side bridge buoy had a broken mounting and was hanging over the deck railing. SOLAS 74 (2014 Cons.) Reg. I/11

The vessel owner or operator must ensure that they maintain one English language copy of the VRP, at a minimum the contents listed in paragraph (c)(1), (2), (3), (5), (6), (7), (9) and (10) of this section and a copy of the Coast Guard approval letter, onboard the vessel. In lieu of paper format, the vessel owner or operator may keep an electronic copy of the VRP and approval letter onboard the vessel. The vessel did not have any of these parts onboard. The vessel had to reach out to its Company and was able to receive them electronically prior to the end of the exam. Vessel personnel unfamiliar with use of VRP. 33 CFR 15.5030

Division – decks, bulkheads and penetrations - The following fire protection systems shall be kept in good working order so as to ensure their required performance if a fire occurs: Structural fire protection, including fire-resisting divisions, and protection of openings and penetrations in these divisions. PSCO observed 02 self-closing fire doors in the vessels accommodation stairwell that would not self-close. SOLAS 74 (2009 Cons.) II-2 Reg 14.2.1

The Administration may permit the use of oil-level gauges with flat glasses and self-closing valves between the gauges and fuel tanks. PSCO observed gauges gaged in the open position on the hydraulic reservoirs by the vessels DP steering system. Crew removed items gagging gauges open. SOLAS 74 (2004 Cons.) II-2 Reg. 4.2.2.3.5.2.2
