The arrangements for the storage, distribution and utilization of oil used in pressure lubrication systems shall be such as to ensure the safety of the ship and persons onboard. PSCO reported significant lubricating and fuel oil leaks throughout the engine room and across all three operational generators. Specifically, excessive oil was observed in the following areas: beneath the aft door in the purifier room, near and around the v/v remote system, leaking from the gauge on the No. 2 M/E F.O. Circ Pump Discharge, under and surrounding the No. 1 H.F.O. Purifier, around and beneath circulator #2, near the lagging of the No. 5 H.F.O. Purifier and underneath the purifier itself. Additionally, fuel had accumulated beneath the covers of each operating generator, with oil pooling on and around each of the generators. 74 SOLAS (2004) II-2/4.2.3.2.3.1

The means for securing and maintaining weathertightness shall be to the satisfaction of the Administration. The arrangements shall ensure that the tightness can be maintained in any sea conditions, and for this purpose tests for tightness shall be required at the initial survey, and may be required at periodical surveys and at annual inspections or at more frequent intervals. PSCO Observed distorted and damaged cargo hatch tie down points on cargo hold #1 port aft, #3 port aft, #4 aft starboard. 1966 ILLC Reg 16 (4)

A hazmat employee must receive the training required by this subpart at least once every three years. PSCO observed the vessels second mate last 49 CFR training was in 2018 and 2017 respectively.

External high-pressure fuel delivery lines between the high-pressure fuel pumps and fuel injectors shall be protected with a jacketed piping system capable of containing fuel from a high pressure line failure. A jacketed pipe incorporates an outer pipe into which the high pressure fuel pipe is placed, forming a permanent assembly. The pressure piping system shall include a means for collection of leakages and arrangements shall be provided with an alarm in case of a fuel line failure. Vessels MDE #5 cylinder has a high-pressure fuel line ruptured and excessive F/O leak. 74SOLAS(09)II-2/4.2.2.5.2 60ac

PSCO observed vessel's MDE had excessive leakage from the cooling water inlet piping on cylinders #3 and #5. 74SOLAS(04)II-1/26.1 60ac

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Vessels starboard lifeboat/rescue boat's battery was dead and it was unable to start. 74SOLAS(24)III/20.2 17c

Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. Fire-extinguisher in the purifier room was found with a broken gauge. Crew replaced extinguisher in kind. 74SOLAS(24)II-2/14.2.1.2 10c

The machinery, boilers, and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board. PSCO observed vessel's MDE had excessive leakage from the cooling water inlet piping on cylinders #3 and #5. Vessel made temporary repairs with a Condition of Class issue until 13JUN25 74SOLAS(04)II-1/26.1 50ac

EACH EMERGENCY GENERATING SET ARRANGED TO BE AUTOMATICALLY STARTED SHALL BE EQUIPPED WITH STARTING DEVICES APPROVED BY THE ADMINISTRATION WITH A STORED ENERGY CAPACITY OF AT LEAST THREE CONSECUTIVE STARTS. A SECOND SOURCE OF ENERGY SHALL BE PROVIDED FOR AN ADDITIONAL THREEWITHIN 30 MIN. UNLESS MANUAL STARTING CAN BE DEMONSTRATED TO BE EFFECTIVE. PSCO OBSERVED SHIP'S CREW TRY TO START EDG WITH HYDRAULIC ACCUMULATOR. EDG DID NOT STQART UPON ACCUMULATION OF THE HYDRAULIC ACCUMULATOR. 74SOLAS(04)II-1/44.2

THE PURPOSE OF THIS REGULATION IS TO PREVENT THE IGNITION OF COMBUSTIBLE MATERIALS OR FLAMMABLE LIQUIDS. FOR THIS PURPOSE, THE FOLLOWING REQUIREMENTS SHALL BE MET: (.1)MEANS SHALL BE PROVIDED TO CONTROL LEAKS OF FLAMMABLE LIQUIDS; (.2)ACCUMULATION OF FLAMMABLE VAPOURS; (.3) THE IGNIGHTABILITY OF COMBUSTABLE MATERIALS SHALL BE RESTRICTED; (.4) IGNITION SOURCES SHALL BE RESTRRICTED; MULTIPLE AREAS OF OIL-SOAKED, DEGRADED LAGGING, OIL LEAKS AND POOLING OBSERVED ON THE MAIN ENGINE BLOCK IN VARIOUS LOCATIONS. 74SOLAS(04) II-2/4.1

THE COMPANY SHOULD ENSURE THAT ALL PERSONNEL INVOLVED IN THE COMPANY'S SAFETY MANAGEMENT SYSTEM HAVE AN ADEQUATE UNDERSTANDING OF RELEVANT RULES, REGULATIONS, CODES AND GUIDELINES. LOCK OUT / TAG OUT PROCEDURES ARE CLEARLY INCORPORATED INTO THE SHIPBOARD SMS, PSCO OBSERVED LOCK OUT / TAG OUT PROCEDURES WERE NOT FOLLOWED FOR INOPERABLE MAIN ENGINE LUBE OIL PURIFIER AND A/E NO. 2 LUBE OIL PURIFIER. 74SOLAS(20) IX/3.1, ISM CODE PART A/6.4

The purpose of this regulation is to prevent the ignition of combustible materias or flammable liquids. Means shall be provided to limit the accumulation of flammable vapours. PSCO observed excessive fuel oil accumulations in the vessel fuel purifier room. 74 SOLAS (2004) II-2/4.1.1

The arrangments for the storage, distribution and uitilization of oil used in pressure lubrication systems shall be such as to ensure the safety of the ship and persons onboard. PSCO observeed excessive lubricating oil leaks throughout engine room and all 4 generators. 74 SOLAS (2004) II-2/4.2.3.2.3.1

A main source of electrical power of sufficient capacity to supply all those services mentioned in regulation 40.1.1 shall consist of at lease two generating sets. PSCO observed that the # 4 generator was the only operating generator available. Vessel's crew was able to start the generator after approximately 30 minutes. 74 SOLAS (2020) II-1/ 41.1.1

An alarm system shall indicate visually each separate alarm function. 02 amber light bulbs on alarm light panels were burnt out in engineering space. 74 SOLAS (20 Cons) II-1/51.1.1

Machinery shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to hot surfaces; steam line under deck plate in F/O purifier room found to be leaking steam and not insulated. 74 SOLAS (so Cons) II-1/26.1

In a ship in which fuel oil is used, the arrangements for storage, distribution, and utilization of the oil fuel shall be as such to ensure the safety of the ship and persons on board. In the oil purified space, PSCO identified a manufactured catch basin attached to the overhead as well as 02 catchbasins under the deck plating containing oil. 74 SOLAS (20) II-2/4.2.2

Means shall be provided to control leaks of flammable liquids. During a Flag inspection, the surveyor identified: "Oil accumulation in the engine room: leaking oil was observed from main engine F.O injection pumps unit No.2, 9,10,11,12" 74 SOLAS (20) II-2/4.1.1

During a Flag inspection, the surveyor identified: "Oil accumulation in the engine room: auxillary engines No. 1,2, and 4 were observed with excessive oil leakages (mainly from F.O. injection pumps), and one F.O high pressure pipe on A/E No. 2 was found broken"

Oil fuel pipes, which, if damaged, would allow oil to escape from storage, settling or day service tank having a capacity of 500L and above situated above the double bottom, shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space. The quick closing valves for the main engine and auxilary generator were blocked open in the oil purifier space.

The main inlets and outlets of all ventilation systems shall be capable of being closed from outside the spaces being ventilated. During a Flag inspection, the surveyor identified: "Fire Safety - ventilation: 2 louvers FWD main engine STBD side were stuck open" 74 SOLAS (20) II-2/5.2.1.1

Following installation, the master, owner, operator, agent, or person in charge of the vessel subject to this subpart must properly maintain the BWMS in accordance with all manufacturer specifications. During a Flag inspection, the surveyor identified:"Other (operation of machinery): Ballast pumps No 1 & 2 were reported non operational" 33 CFR 151.2025(a)(1)

During a Flag inspection, the surveyor identified: "Cleanliness of engine room: overall engine room housekepping and cleanliness was found in poor condition (several spare sparts were seen not secured, several oil leakages were seen around main engine cylinder heads)"

During a Flag inspection, the surveyor identified: "Oil accumulation in the engine room: oil leakage was identified from G/E LO storage tank quick closing valve, and from the sight glass of M/E cylinder LO storage tank." 74 SOLAS (20) II-2/4.3.1

During a Flag inspection, the surveyor identified: "Other (machinery): Water leakage was identified from settling tank No. 2 heating valve." 74 SOLAS (2) II-1/26.1

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that in all respects, that the ship remains fit to proceed to sea without danger the ship or persons on board. During a Flag inspection, the surveyor identified: "Other (machinery): One S.W pipe for fire pump was seen cracked and leaking water into the bilges, and on SW pipe above main air compressors was seen leaking with temporary collecting arrangement is used" 74 SOLAS (20) I/11(a)

Following installation, the master, owner, operator, agent, or person in charge of the vessel subject to this subpart must properly maintain the BWMS in accordance with all manufacturer specifications. During a Flag inspection, the surveyor identified: "Ballast Water Treatment System doosing pumps were found leaking in lower engine platform" 33 CFR 151.2025(a)(1)

After survey of the ship… has been completed, no change shall be made… to equipment without the sanction of the Administration. During a Flag inspection, the surveyor identified: "Other (fire safety): Hand wheel for isolating valve of main engine turbo charge LO pump No. 2 was found missing"

The company should establish procedures to ensure to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations. In meeting these requirements the company should ensure that inspections are held at appropriate intervals, any non-conformity is reported with its possible cause, if known; appropriate corrective action is taken, and records of these activities are maintained. The technical and operational nature of the deficiencies noted above both by the Coast Guard and the Administration, provide evidence of a serious failure of the implementation of the ISM Code. An external audit is required within three months. 74 SOLAS (20) IX/3 ISM Code 10.2

74 SOLAS (20) II-2/14.1.1 "...the following functional requirements shall be met: fire protection systems and fire-fighting systems and appliances shall be maintained ready for use. PSCO received a report from the vessel that the water mist system is malfunctioning in the boiler and incinerator area." 60ac

The following fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs: structural fire protection, including fire resisting divisions, and protections of opening and penetrations in these divisions. PSCO discovered 03 inoperable self-closing fire doors. 01 was located on the bridge and fails to self-close. 01 was located at a water closet on the upper deck and fails to self-close. 01 was located at a locker room leading to the engine room, fails to self-close, and is bent. 74 SOLAS (04)/ 14.2.1.1 17ac

The vessel experienced a grounding while transiting the navigable waters of the United States.
