The water spray system cooling, fire protection and crew protection should be installed to cover exposed cargo tank domes and every exposed tank. Eight sprinkler heads did not supply water to all parts of the cargo tank. SOLAS 74 9consolidated 2020) Ch. VII/Reg. 13 IGCC 11.3.1.1

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons onboard. Port side deck 2 watertight door across from wiper room did not seal properly as required.
