Ships shall carry at least 2 fire fighter outfits & 2 additional outfits on tankers. Fighters outfits shall consist of protective clothing of material to protect skin from heat radiating from fire & from burns & scalding by steam. The vessel has four fire fighter outfits on board. One suit is missing three snaps, preventing closure of jacket, leaving crew members skin exposed to heat & flame.

PSCO observed missing wire net on 02 out of 04 fuel tank vents on No. 01 Fuel Oil Tank (fwd, FR 182-188). As per the vessel schematics onboard all oily tank vents shall have wire net installed on vents. 17ac - 74SOLAS14 CH1 Reg. 11

Firefighting outfits shall comply with the Fire Safety Systems Code. The firefighting suits shall protect the skin from radiating heat and the outer surface shall be water resistant. PSCO observed one suit in which the pants have worn and had lost its water resistant material. On an additional suit the pair of gloves did not have water resistant material on the seam. 17ac - 74SOLAS14 CH II-2 Reg. 10.1

Where "B" class divisions are penetrated for passage of electrical cables, pipes, trunks, ducts ets. Arrangement shall be made to ensure that the fire resistance is not impaired. PSCO observed an after survey installation of WiFi cables in the electrical and cable space on A deck. As well as two hole penetrating the bulkhead above the door. The WiFi cables and holes were not filled with an intumescent material, calling into question the fire resistance of the space. 17ac - 74SOLAS14 CH II-2 Reg. 9.3.3.2

Food waste was found mixed with other garbage and paper at the final collection point prior to being discharged into the sea.
