All owners/operators of vessels must ensure that the master, operator, person-in-charge and crew members who actively take part in the management of incidental discharges...Are adequately trained in implementing the terms of this permit. After operational test of vessels fire monitors, crew members pulled vessel scupper plugs and did not collect drainage for proper treatment in accordance with VGP procedures.

A Chemical Tanker shall comply with the requirements of the International Bulk Chemical Code. Samples kept on board shall be stowed in a designated space and shall be cell-divided in order to avoid shifting of the bottles at sea. Vessel does not have adequate stowage arrangements for samples while at sea. PSCOs found broken samples while inspecting sample locker.

Cheif Officer conducted a flow-through exchange during arrival to Sector Delaware Bay COTP zone.
