All owners/operators of vessels must ensure that the master, operator, person-in-charge and crew members who actively take part in the management of incidental discharges...Are adequately trained in implementing the terms of this permit. After operational test of vessels fire monitors, crew members pulled vessel scupper plugs and did not collect drainage for proper treatment in accordance with VGP procedures.

Cylinders must always be secured and, when not in use, they must be stowed in a rack in an upright position with the valve protection cap in place. Vessel placed 4 cylinders horizontally on the deck with no rack for securing in place.

A Chemical Tanker shall comply with the requirements of the International Bulk Chemical Code. Samples kept on board shall be stowed in a designated space and shall be cell-divided in order to avoid shifting of the bottles at sea. Vessel does not have adequate stowage arrangements for samples while at sea. PSCOs found broken samples while inspecting sample locker.

The master, owner, operator, agent, or person-in-charge of any vessel equipped with Ballast Water tanks that operates in the waters of the United States must follow these practices: maintain a Ballast Water Management Plan that has been developed specifically for the vessel and that will allow those responsible... Tor understand and follow the vessels strategy...Vessels Ballast water management plan indicates that vessel should employ a sequential method for ballast water exchange. Cheif Officer conducted a flow-through exchange during arrival to Sector Delaware Bay COTP zone.
