FIRE DETECTORS SHALL BE TESTED ACCORDING TO STANDARDS EN-E4-10:2001 AND IGC 60092-504. CREW DOES NOT HAVE APPROVED EQUIPMENT FOR FIRE DETECTOR TESTING ONBOARD.

NOZZLES SHALL BE OF AN APPROVED PURPOSE TYPE (I.E. SPRAY/JET TYPE) INCORPORATING A SHUTOFF. THE NOZZLES ONBOARD WERE TESTED BY [name withheld] WOULD NOT PROPERLY TRANSITION FROM JET.

The arrangements provided shall be such as to ensure that the safety of the ship in all sailing conditions, including maneuvering, is equivalent to that of a ship having the machinery spaces manned. Measures shall be taken to the satisfaction of the Administration to ensure that the equipment is functioning in a reliable manner and that the satisfactory arrangements are made for regular inspections and routine tests to ensure continuous reliable operation. Vessel's Main Engine Control Panel and valve operation monitoring system are partially inoperable. Vesel currently depends on one monitoring screen for visual identification of parameters which restricts its ability to operate minimally manned. (74 SOLAS (04) II-1/46.1&2)

Main and emergency switchboards shall be so arranged as to give easy access as may be needed to apparatus and equipment, without danger to personnel. The front of switchboards shall be sufficiently guarded. Main switchboard has several newly installed breakers which differ in size from original breakers. These have created open slots to the feeding power rails which creates access to live electrical hazards. (74 SOLAS (04) II-1/45.2)

A fixed fire detection and fire alarm system shall be installed in periodically unattended machinery spaces. Forward fire detector head over the main engine was observed missing. (74 SOLAS (04) II-2/7.4.1.1)

PSCO observed both of the vessel's fire fighter outfits to be fraying around the neck and underarms. Making them inadequate to protect skin from the heat of a fire. 74SOLAS(04) II-2/10.2.1

The annual surveys referred to in paragraph (1)(c)of this article shall be endorsed on the International Load Line Certificate. The vessel's curretn International Load Line Certificate does not mention the last annual survey. Cite: ICCL 66/88 Article 14(2).

The condition of the ship and it's equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons onboard. When the Port State Control team witnessed the emergency and main steering test, a 5 degree deviation was onserved and a hydraulic lock alarm was activated as the team tested the #2 steering gear pump. Main source of electrical power shall consist of at least two generating sets. Two of the ships three generators are down for maintenance. 74SOLAS(04)II-1/40.1.1

Oil filtering equipment shall be provided with alarming arrangements. The accuracy of equipment shall be verified suring IOPP Certificate renewal. The calibration certificate was unable to be produced at the time of inspection however master did locate and send over via email. MARPOL I/13.7 MEPC 107(49)4.2.11

Cables and wiring shall be installed and supported in such a manner as to avoid chafing or other damage. Lighting fixture on port side cargo deck at the stairwell to the poop deck has themetal support that is no longer welded to the deck. The wires and support are dangling and could chaff causing an electrical shock hazard. SOLAS 09 II-1/Reg 45.5.5

Lifebuoys shall be so distributed as to be readily available and so stowed as to be capable of being rapidly cast loose. Both port and starboard forecastle lifebuoys were not stowed in their assigned locations.

After any survey of the ship under regulation 7,8,9 or 10 had been conducted, no change shall be made....Alteration has been made to wiring running from the radar to bridge through penetration in bulkhead.

Doors required to be self closing shall not be fitted with hold back hooks. Multiple self closing A class boundaries are fitted with rope to tie the boundary open. SOLAS II-2/Reg 9.4.2.2

Maintenance, testing and inspections shall be carried out based on teh guidelines developed by the organization and in a manner having due regard to ensuring the reliability of fire fighting systems and applicances. CO2 bottles throughout the vessel have exceeded the maintenance cycle set by the administration. SOLAS 09 II-2/Reg 142.2.1

The purpose of the regulation is to prevent the ignition of combustible materials or flammable liquids. For this purpose the following shall be met...ignition sources shall be seperated from combustible materials. Old papers are being stored in direct contact with control panel for bridge lighting.

The following fire protection systems shall be kept in good working order: structural fire protection, including fire/A class resisting divisions. Multiple doors throughout the ship were identified as self closing but do not close on their own. SOLAS 09 II-2/Reg 14.2.1.1

No change shall be made in the structural arrangements, machinery, equipment or other items without the sanction of the Administration. A hose has been arranged from the condensation output on the AC unit, thru the wall via small hatch, and is so arranged as to discharge directly overboard. SOLAS 09 I/Reg 11(a)

Drills shall as far as practicable be conducted as if there were anactual emergency. While crew had knowledge of proceedures and required equipment, no one grabbed any required items as per S.O.P. SOLAS 09 III/Reg 19.3.1

The galley door and stairwell doors are marked as being A-0 doors on teh fire control plan. A-15 doors are fitted into these locations. Door located at the bottom of teh stairwell is warped and does not close at the bottom, the door handle does not function properly, and the top of the door has been drilled thru in order to secure the closing mechanism which negate the fire integrity of this door.

Operating instructions, including block diagram, for steering gear were not posted on navigating bridge and in steering gear compartment as per SOLAS 74 (amended) Chapter V/26.3.1. Correct to satisfaction of class prior to next US port after sailing foreign.

Vessel does not have on-board a vessel-specific ballast water management plan as per 33 CFR 151.2035(a)(7).

Vessel is not maintaining ballast water recordkeeping in accordance with 33 CFR 151.2045; missing total ballast water information including number of tanks and location of exchange.

During rescue boat drill crew was unable to start rescue boat after 20 minutes of attempts. Rescue boat was not in a state of continuous readiness for launch within 5 minutes as per SOLAS 74 (amended) Chapter III/14.1. SOLAS 74 (amended) III/20.2.

Only copy of SOLAS training manual required by SOLAS 74 (amended) III/35 was kept on bridge, not in each crew messroom, recreation room, or cabin. Master moved manual to crew's messroom until such time as copies are placed in officer messroom and recreation room. SOLAS 74 (amended) III/35.2.
