Electrical equipment on a vessel must be installed and maintained to: Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Observed exposed electric cables for thruster in the portside engine room. Rectify cables condition to the satisfaction of the MI. (46 CFR 183.200(b))

Piping, including valves, pipe fittings and flanges, conveying vapor, gas, or liquid, the temperature of which exceeds 65.5 degrees Celsius (150 degrees Fahrenheit), must be suitably insulated where necessary to prevent injuries. Observed the exhaust lagging was deteriorated, leaving hot piping exposed in the portside engine room. Replace exhaust lagging and cover the hot pipe surface to the satisfaction of the MI. (46 CFR 177.970)

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Observed a combination of both child and adult life jackets in the same starboard life jacket box. Separate and clearly mark their different locations to the satisfaction of the MI. (46 CFR 180.78)(a)(5))

Each life jacket must be marked in clearly legible block capital letters with the vessel's name. Observed that a number of life jackets were marked "REAL MCCOY" or without any marking. Properly mark life jackets "REAL MCCOY II" to the satisfaction of the MI. (46 CFR 185.604)

A vessel must carry one floating waterlight, unless it is limited to daytime operation, in which case no floating waterlight is required. Observed that both life ring buoy's waterlight batteries were expired. Replace batteries to the satisfaction of the MI. (46 CFR 180.70(d)(1))

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Observed that the FCC Radio Station Authorization was expired. Replace with new, valid certificate to the satisfaction of the MI. (46 CFR 184.502)

The master shall have appropriate "NO SMOKING" signs posted and shall take all necessary precautions to prevent smoking or carrying of lighted or smoldering pipes, cigars, cigarettes, or similar items in the deck area assigned to automobiles or other vehicles. Observed that the signage on the starboard side was severely faded. Replace sign to the satisfaction of the MI. (46 CFR 185.340 (c))

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Vessel reported they collided with an unknown object causing the steering motor shaft to break. Vessel restricted from movement until attending MI conducts satisfactory ISE and witnesses operational test of propulsion system. 46 CFR 182.200(a)

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Vessel experienced a loss of propulsion on both engines, resulting in an unintentional grounding. Provide root cause, corrective actions, and detailed damage assessment to the satisfaction of the MI. 46 CFR 182.200(a)

For fixed-gas fire extinguishing systems, the inspections and tests required by Table 176.810(b) of this section, in addition to the tests required by 46 CFR 147.60 and 147.65. The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. Vessel unable to provide satisfactory firefighting servicing report for fixed-Co2 systems. Provide firefighting servicing report for #1 ENG space and #2 ENG space Co2 systems to satisfaction of attending marine inspector. Provide servicing report within 7 days.

Each portable fire extinguisher on a vessel must be of an approved type. The minimum number of extinguishers required on a vessel must be acceptable to the OCMI, but must not be fewer that the minimum number required by Table 181.500(b). Pilot house portable extinguisher missing. Provide proper extinguisher within 7 days.

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. Marine inspector observed expired medications and eyewash in first aid kit on board the vessel.

Each life jacket must be marked with Type I retroreflective material approved in 46 CFR 164.018. Several adult lifejackets had failing retro tape that needs repair.

For fixed-gas fire extinguishing systems, the inspections and tests required by Table 176.810(b) of this section, in addition to the tests required by 46 CFR 147.60 and 147.65. The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. Vessel unable to provide satisfactory firefighting servicing report for fixed-Co2 systems. Provide firefighting servicing report for #1 ENG space and #2 ENG space Co2 systems to satisfaction of attending marine inspector. 46 CFR 176.810 (b) (2)

Each ring life buoy must be marked in clearly legible block capital letters with the vessel's name. 02 of vessels ring life buoys are missing vessels name. 01 ring life buoy is faded. Provide proper marking of vessel name on each ring life buoy. 46 CFR 185.604 (b)

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of machinery including the following: inspection of all machinery essential to the routine operation of the vessel. Associated operation manuals for HUG Diesel Particulate Filter (DPF) all state hard copies of manuals must be near the system at all times. Manuals for system were not available onboard. Provide required copies of all manuals onboard. 46 CFR 176.804

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of machinery including the following: inspection of all machinery essential to the routine operation of the vessel. Operations manuals for HUG DPF system refer to maintenance requirements and state certain functions (A work) must be done by HUG Engineering AG or an authorized partner. Maintenance records were not available for inspection. Provide maintenance records and qualifications of personnel conducting any maintenance A work. 46 CFR 176.804

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of machinery including the following: inspection of all machinery essential to the routine operation of the vessel. HUG DPF system onboard was commissioned to run with regeneration burner automatically running burning cycle at 15 hours. Crew stated DPF system is unable to auto run due to hydraulic demands of DPF in addition to other hydraulicly powered systems onboard. Operators currently have mechanics come onboard bi-weekly to manually run regeneration burner. Provide records of bi-weekly regeneration operation and details in accordance with manufactures instructions that this is a suitable method for operating these units. 46 CFR 176.804

Each lighting fixture globe, lens, or diffuser must have a guard or be made of high strength material. Light above #1 ENG has broken guard. Install proper guard to satisfaction of Marine Inspector. 46 CFR 183.410

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of machinery, fuel, and piping systems, including the following: (a) operation of the main propulsion machinery both ahead and astern. Vessel unable to complete underway portion of annual inspection due to maintenance of #1 ENG coolant line. Complete satisfactory underway portion of annual inspection, including Man Overboard drill to satisfaction of attending Marine Inspector. 46 CFR 176.804 (a)

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Prior to inspection, Master of the vessel discovered a small leak on #1 ENG coolant piping via associated reduced pressure alarm. Vessel completed repairs to satisfaction of attending Marine Inspector. 46 CFR 182.200 (a)

The frame or suppoer of each semi-portable fire extinguisher permitted by paragraph (a) of this section must be welded or otherwise permantently attached to a bulkhead or deck. MI discovered 02 portable fire eztinguishers not properly mounted. 46 CFR 181.500(d)

Employers must maintain records of chemical tests and must make these records available to the Coast Guard upon request. Vessel operator noted that the records were kept in their business office in Sacramento, not accessible for the inspectors to review during the inspection.

A vessel must carry a first-aid kit approved under approval series 160.041 or a kit with equivalent contents. Vessel's kit was missing ammonia inhalants and aspirin.

All Machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure and personnel injury. Vessel’s service generator was not present at the time of its annual inspection.

If an individual holding a credential fails a chemical test for dangerous drugs, the individual's employer must report the test results in writing to the nearest Coast Guard Officer in Charge of Marine Inspection (OCMI). CAL TRANS internal policy stated all mariners who receive positive chemical tests were placed into company-inposed treatment and rehabilitation program prior to being placed back into a safety sensitive position. Notification to USCG was not taking place. Therefore, the mariner was not subject to suspension and revocation proceedings against his or her credential under 46 CFR part 5.

At every vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Uncontrolled leak of hydraulic fluid from failed O-ring and fitting on the "A" line of the #1 main diesel engine. Vessel prohibited to carry passengers for hire until satisfactory operational tests are observed by USCG Marine Inspector.

A vessel must be operated in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons being transported. Vessel crew have submitted numerous inspection reports over the previous months to owner/operator stating consistent hydraulic leaking from #1 thrustmaster at s/s block where the affected fitting and o-ring are located. Master notified company of ongoing leaks and maintenance failures within the "A" hydraulic line. Company did not provide any repair or maintenance oversight despite frequent daily inspection reports from crew. Crew recognized inherent danger for pollution and potential loss of propulsion due to drop in hydraulic pressure. Crew forced to mitigate risk to vessel, personnel and environment by installing absorbent pads around the leaks. In spite of crew submitted inspection reports and attempts to mitigate the failure, catastrophic failure occurred on the #1 main propulsion engine "A" hydraulic line fitting and O-ring. Preliminary review of crew-submitted daily vessel inspection reports to company show the failures within the hydraulic lines have been known since at least November 6th 2018.

Repair Fire/ bilge pump pilot line of the pressure compensating valve for the auxiliary hydraulic valve stack. 46 CFR 181.300

Instrumentation cable bundling, in both engine rooms, is zip tied to and resting on the main engines. The cabling must be hoisted and supported by a metal bracket. 46 CFR 183.340(b)(4)

Vessel has significant leakage from #1 propeller swivel block and requires effective repairs IAW 46 CFR 182.100.

Vessel's auxiliary hydraulic pump leaks fluid and requires repair IAW 46 CFR 182.100.

Prove proper operation of azimuthing drive system following repairs.

Provide repair description for failed hydraulic system components. Prove proper operation of system after completion of repairs in the presence of a marine inspector, in accordance with 46CFR176.700(a)(d)

Loss of propulsion/steering on #2 MDE due to hydraulic accumulator. Remove port and starboard accumulators, reconfigure hydraulic system as originally designed, prove proper operation of port & starboard propulsion and steering systems under power.

Provide plan proposal for accumulators with safety mechanism to prevent accidental loosening from vibration, such as safety wire, for review prior to installation.
