Each vessel connection flange face must have a permanent stud projecting outward that has a 12.7 millimeter (0.5 inch) diameter and is at least 25.4 millimeter (1 inch) long. It must be located at the top of the flange face, midway between boltholes, and in line with the bolthole pattern. PSCO observed that #1 aft and #2 forward Starboard vapor lines were missing the required stud.46CFR39.2001(j)

1910 - A tank from which a substance in category X has been unloaded shall be prewashed before the ship leaves the port of unloading. Appropriate entries of these operations shall be made in the Cargo Record Book and endorsed by the surveyor. Cargo tanks 9P and 9S unloaded Trimethyl Benzene, all isomers and category X cargo, on 17DEC17 at Kinder Morgan Deer Park in Houston, TX. A prewash was conducted but no survey was conducted by an authorized surveyor to verify the concentration of cargo in the tank washings. The tanks were subsequently washed at sea on 18DEC17 and loaded with acrylonitrile, a compatible category Y cargo on 23DEC17 (9S and 26DEC17 (9P).
