Each vessel connection flange face must have a permanent stud projecting outward that has a 12.7 millimeter (0.5 inch) diameter and is at least 25.4 millimeter (1 inch) long. It must be located at the top of the flange face, midway between boltholes, and in line with the bolthole pattern. PSCO observed that #1 aft and #2 forward Starboard vapor lines were missing the required stud.46CFR39.2001(j)

1860 - Each self propelled ship must have the following: Two stretchers or wire baskets complete with equipmentfor lifting an injured person from a pump room or a cargo tank. The stretcher in the ballast pump room was ripped and torn and would not be able to safely lift a person.
