PSCO's observed excessive fuel leakage on tow oil purifers and the main fuel supply, resulting in soaked laggin and build up.

Resolution: The system was maintained the leaking was rectified, the leakage was cleaned by
crew. The contaminated lagging were replaced in the area mentioned.
The other contaminated lagging identified during class annual survey was issued as CC.

Resolution: The system was inspected by service engineer and suspected the antenna damage.
Then new NAVTEX system was installed by service engineer and confirmed in order.

Every ship shall be provided with a receiver capable of receiving international Navtex service broadcasts if the ship is engaged on voyages in any area in which an International Navtex Service is provided.
PSCO observed "No Data" message. PSCO requested crew show that Navtex could received messages for it's intended use. After several failed attempts, crew stated that Navtex was not receiving messages

Resolution: The three doors were repaired by crew, the function test of door was witnessed by attending surveyor
and found in order.
The other fire doors were also inspected during annual survey and found in order.
