THE VESSEL FAILED TO FULLY ESTABLISH PROCEDURES TO ENSURE THE SHIP IS MAINTAINED IN CONFORMITY WITH THE PROVISIONS OF THE RELEVANT RULES. PSCO observed excessive fuel oil leaking of coupling on top of filter assembly in fuel purifying system. Oil was pooled on top of unit. SOLAS(04) 11-2 4.1.1

For this purpose, the following requirements shall be met: means shall be provided to control leaks of flammable liquids. PSCO observed fuel oil leaking excessively at X ring of piston pump in way of fuel oil filter assembly. Oil leaked into drip pan and was being emptied by ships crew. SOLAS(04) II-2 4.1.1

Before the ship leaves port and at all times during the voyage, all life saving appliances shall be in working order and ready for immediate use. PSCO observed that port side life rafts did not have painter lines attached to the weak link of hydrostatic release. SOLAS(14 amended) III 20.2
