Lighting fittings should have pressurized enclosures or should be of the flameproof type. PSCO observed cracked and damaged explosion proof lighting within hazardous area including 2 in SOPEP locker, 8 in compressor room and 6 in motor room. Additionally, PSCO observed damaged electrical box at starboard embarkation ladder.

Gas detection equipment should be so designed that is may be readily be tested. Vessel's fixed gas detection flow failure alarm was non-operational at time of testing.

On Ships carrying flammable oil toxic products, a water-spray system should be installed capable of covering all areas on cargo tank domes cargo liquid and vapor loading and discharge manifolds, and boundaries of superstructures and deck holes normally manned. During the testing of the deck water-spray system the psco's noted that the port side manifold, and superstructure areas were not properly covered. In addition, there were 18 spray nozzles clogged at the cargo independent deck tank. Vessel is prohibited from conducting cargo operations until satisfactory test of deck water spray system.
