Vapour detection system in acid room were covered and taped. Have tape removed and prove function. IBC 1.5.3.1

The company should establish procedures, plans, and instructions, including checklist as appropriate for key shipboard operations concerning the safety of the personnel, ship, and protection of the environment. The company shall also ensure any non-conformities are reported, with its possible cause and take appropriate action. The gas detection system was discovered with the low level alarm flashing and the attached sounding horn was silenced due to the detection. There is no checklist or notification procedures in place other than a bridging document for these discoveries.

Means shall be provided to control leaks of flammable liquids and limit accumulation of flammable vapors. While conducting inspection of frack package onboard aft deck, Inspectors discovered significant amount of hydraulic fluid, water, and frack chemical mixture in bilges by mixing room w/ tanks and aft port bilge in pump room. Master shall ensure that contract company removes flammable liquids from bilge to satisfaction of ABS/CG. 74SOLAS(14) II-2/4.1
