No electrical equipment shall be installed in paint lockers, unless the administration is staisfied that such equipment is appropriately certified for safe usage in the dusts, vapours, or gases likely to be encountered.
Cable connection to explosion proof lighting in paint locker was found to be loose, exposing the internal of the lighting to the atmosphere.

Where "A" Class divisions are penetrated, such penetrations shall be suitably insulated by extension of the insulation at the same level of the division.
Thru deck pipe run between A/C Room and engine room was missing insulation and exposed an "A" class division. A rag was stuffed in the fitting. Crew removed the rag and propertly insulated with cable sealing.
Condition: Design Flaw
