Vessels of 12 meters or more in length but less than 50 meters require an all-round light. MI observed missing all-round light. Provide all-round light. 705c: Vessel restricted to daylight operations and limited to 2nm of visibility. 33CFR83.22(b)(v)

Paints, coatings, or other flammable or combustible products onboard a towing vessel must be stored in a designated storage room or approved cabinet when not in use. MI observed wasted corners on cabinet used for storage. Remove or Replace. 46CFR142.225 16c

Towing vessels of 65 feet or less in length must carry at least the minimum number of portable fire extinguishers set forth in table 142.230(a). MI observed 02 extinguishers requiring recharge and a wasted mounting bracket. Provide required number of extinguishers in approved mounting brackets.

Closure devices must be provided for deckhouse or hull penetrations, which open to the exterior of the vessel and which may allow water to enter the vessel. MI observed failed gasket on pilothouse securing hatch. Replace gasket. 46CFR144.320(b) 16c

No later than July 22, 2019, the owner or managing operator must implement a health and safety plan. The health and safety plan must document compliance with this part and include recordkeeping procedures. Health and safety was not present. 46 CFR 140.500(a) - 16C

Means must be provided for stopping each fan in a ventilation system serving machinery spaces and for closing, in case of fire, each doorway, ventilator, and annular space around funnels and other openings into such spaces.

Each vessel must be designed and constructed to minimize fire hazards insofar as reasonable and practicable. MI observed the sight glass for the port fuel tank missing guards or protection from breakage. 46CFR144.405

Means must be provided for stopping each fan in a ventilation system serving machinery spaces and for closing, in case of fire, each doorway, ventilator, and annular space around funnels and other openings into such spaces. MI observed the engine room intake vent cover to be deteriorated and the exhaust vent for the engine room to have no cover for closing. Repair engine room intake cover and install engine room exhaust vent cover to satisfaction of MI. 46CFR144.605

Paints, coatings, or other flammable or combustible products onboard a towing vessel must be stored in a designated storage room or cabinet when not in use. If a dedicated storage cabinet is provided it must be secured to the vessel so that it does not move and must be either:A flammable liquid storage cabinet that satisfies UL 1275 or a flammable liquid storage cabinet that satisfies FM Approvals Standard 6050. MI observed flammable liquids being stored on the open deck. Provide approved type storage cabinet of sufficient size meeting either of the specific requirements. 46CFR142.225

All portable fire extinguishers on board the vessel must be inspected or tested at least once every 12 months, as prescribed in paragraphs (a)(1) through (8) of this section. MI observed the portable fire extinguisher located starboard side on deck did not have annual servicing conducted along with the pin to be wasted and seized. Additionally, portable fire extinguisher located in the pilot house had inadequate mounting bracket for the size of the extinguisher. 46CFR142.240(a)

Each lifebuoy on a towing vessel must: Be approved under approval series 160.050 or 160.150. MI observed the lifebuoy on the starboard side of the pilot house to have a deteriorated grab line and insufficient retro-reflective tape. Repair grab line and install new retro-reflective tape where needed to the satisfaction of MI. 46CFR141.360(b)(1)

Decks and bulkheads designed to be watertight or weathertight must be maintained in that condition. Marine Inspector observed that 04 bulkhead penetrations in the vessel forepeak were not sealed/watertight. 46 CFR 140.610(f)

Except as provided in paragraph (a)(8) of this section, each towing vessel must have a fire-detection system installed to detect engine room fires. The owner or managing operator must ensure the following: The system draws power from two sources. Switchover from the primary source to the secondary source may be either manual or automatic. The fire detection system emergency battery power source was inoperable when tested. 46 CFR 142.330

A towing vessel must carry a combination of day and night visual distress signals indicated in Table 141.370 of § 141.370 for specified areas where the vessel operates. Hand-held red flare distress signals, approved under approval series 160.021 or 160.121, and hand-held rocket-propelled parachute red flares, approved under approval series 160.036 or 160.136, are acceptable as both day and night signals. Found unapproved distress signals onboard. 46 CFR 141.375

Closure devices must be provided for deckhouse or hull penetrations, which open to the exterior of the vessel and which may allow water to enter the vessel. These devices must be suitable for the expected route. Identified a wasted weathertight door at the entrance to the deckhouse on the main deck and missing gasket material at the stern entrance to the engine room. Replace the weathertight door at the entrance to the deckhouse and replace the gaskets in the engine room hatch. 46 CFR 144.320

Each towing vessel must carry on board and maintain the following: Marine charts or maps of the areas to be transited, published by the National Ocean Service (NOS), the ACOE, or a river authority that satisfy the following requirements: (i) The charts or maps must be of a large enough scale and have enough detail to make safe navigation of the areas possible. The charts or maps must be either - (A) Current editions or currently corrected editions, if the vessel engages in towing exclusively on navigable waters of the U.S., including Western Rivers; or (B) Currently corrected editions, if the vessel engages in towing seaward of navigable waters of the U.S. or more than three nautical miles from shore on the Great Lakes. Nautical charts required for intended voyage were not onboard. 33 CFR 164.72

A COI may be suspended and withdrawn or revoked by the cognizant Officer in Charge, Marine Inspection at any time for noncompliance with the requirements of this subchapter. Upon administrative review, the vessel is not permitted to operate outside of Routes and Conditions dictated in COI unless a new COI is issued for a Lakes, Bays, and Sounds route which is the designated route the vessel intends to operate on in Tampa Bay, FL. 46 CFR 136.215(c)

A towing vessel subject to this subchapter and choosing the Coast Guard inspection option, or required to have the Coast Guard inspection option, must undergo an annual inspection within 3 months before or after the COI anniversary date. Upon administrative review, MI found vessel to be past due date for annual inspection. Vessel was required to complete annual inspection by 18FEB21 and 18FEB22. COI is no longer valid. 46 CFR 137.200
